摘要
目的分析与评价临床医学专业毕业生客观结构化临床考试(OSCE)方法,为进一步完善OSCE和开展医学生临床能力评价提供依据。方法分析了2005—2019年临床医学专业毕业生OSCE的特点,并在2019年考核后,向毕业生和考官(教师)发放调查问卷进行OSCE评价。结果2005—2019年3个不同阶段,OSCE考站越来越多,考核内容越来越全面,考试时间越来越长,评分标准越来越严格,对考官(教师)的培训越来越规范,且逐渐增加了对医学生有关OSCE的指导,但不同阶段OSCE均缺乏考核后的反馈环节。90%以上考官(教师)和医学生认为OSCE优于传统的考核方式,具有较高的实用性和可操作性,可以充分反映医学生的临床胜任力;74.4%医学生认为OSCE站点多、难度大、考核时间长,但可以接受;20%左右考官(教师)和医学生认为OSCE不能充分反映职业态度和医学人文素养。虽然88.5%的考官(教师)掌握了OSCE考核方法与技巧,但所有考官(教师)均认为仍需加强OSCE培训。结论OSCE是一种有效的考核临床胜任力的方法,但应进一步完善考站设计、加强考前指导、增加考后反馈以及强化考官(教师)培训,以更好地发挥OSCE的作用。
Objective To analyze and evaluate the objective structured clinical examination(OSCE)method in graduates majoring in clinical medicine,and to provide a basis for further improving OSCE and evaluating the clinical skills of medical students.Methods The characteristics of OSCE were analyzed for the graduates majoring in clinical medicine in 2005-2019,and a questionnaire was distributed to graduates and examiners(teachers)for OSCE evaluation after assessment in 2019.Results During the three different stages from 2005 to 2019,there was an increase in the number of OSCE test stations,and the contents of the examination became more and more comprehensive,with a longer examination time and stricter scoring standards.The training of examiners(teachers)was more standardized,with an increase in the guidance on OSCE for medical students,but there was a lack of feedback after examination in the different stages of OSCE.More than 90%of the examiners(teachers)and medical students believed that OSCE was better than the traditional assessment methods and had high practicability and operability,which could fully reflect the clinical competence of medical students;74.4%of the medical students believed that OSCE had many test stations,was difficult,and had a long assessment time,which were all acceptable;about 20%of the examiners(teachers)and medical students believed that OSCE did not adequately reflect professional attitudes and medical humanities.Although 88.5%of the examiners(teachers)mastered the assessment methods and skills of OSCE,all of them believed that it was necessary to further strengthen OSCE training.Conclusion OSCE is an effective method to assess clinical competence,but it is necessary to further improve test station design,strengthen pre-examination guidance,add post-examination feedback,and enhance the training of examiners(teachers),so as to give full play to the role of OSCE.
