摘要
目的探讨小剂量甲泼尼龙片联合穿虎祛痛合剂治疗非甾体抗炎药和秋水仙碱禁忌的难治性痛风效果。方法报告1例应用小剂量甲泼尼龙片联合穿虎祛痛合剂治疗的难治性痛风病人的临床资料。结果该病人局部疼痛经3 d局部应用糖皮质激素联合穿虎祛痛合剂治疗后好转;经2周口服小剂量甲泼尼龙片联合穿虎祛痛合剂治疗后剧烈疼痛等症状缓解,炎症指标及肝功能恢复正常。2周后停用甲泼尼龙片,痛风未发作;1月后行肘关节痛风石手术治疗时,痛风石容易取出。结论小剂量甲泼尼龙片联合穿虎祛痛合剂是肝功能异常的难治性痛风病人的可选择治疗方案。
Objective To investigate the efficacy of low-dose methylprednisolone tablet combined with Chuanhu Qutong mixture in the treatment of refractory gout with contraindications to nonsteroidal anti-inflammatory drugs and colchicine.Methods This article reported the clinical data of one patient with refractory gout who were treated with low-dose methylprednisolone tablet combined with Chuanhu Qutong mixture.Results In this patient,local pain was improved after 3 days of treatment with glucocorticoids combined with Chuanhu Qutong mixture.After 2 weeks of oral administration of low-dose glucocorticoids combined with Chuanhu Qutong mixture,the symptoms including severe pain were alleviated,and inflammation markers and liver function returned to normal.Glucocorticoids were stopped 2 weeks later,and gout attack was not observed.The tophi was easily removed at the time of elbow tophi surgery 1 month later.Conclusion Low-dose methylprednisolone combined with Chuanhu Qutong mixture is an optional treatment regimen for patients with refractory gout and abnormal liver function.
