摘要
目的对比Xpert MTB/RIF与改良罗氏比例法药敏试验检测利福的平药敏结果。方法选取2019年7月—2022年4月淮安市第四人民医院住院或者门诊中同时进行Xpert MTB/RIF和改良罗氏比例法药敏试验检测利福平药敏的246例肺结核患者为研究对象,比较两种方法检测利福平的药敏情况。结果Xpert MTB/RIF和改良罗氏比例法药敏试验检测利福平的耐药率比较,差异无统计学意义(χ^(2)=1.15,P>0.05)。比例法药敏试验检测敏感,Xpert MTB/RIF耐药有8例;比例法药敏试验检测耐药,Xpert MTB/RIF敏感0例。246份标本经Xpert MTB/RIF检测出利福平耐药有36例,在结核杆菌RRDR设计ProbeA、ProbeB、ProbeC、ProbeD、ProbeE 5条探针中,Xpert MTB/RIF系统检测利福平耐药位点以ProbeE最高63.9%。以改良罗氏比例法药敏试验为金标准Kappa=0.86。结论Xpert MTB/RIF检测利福平的药物敏感性用时短,高通量,灵敏度高,特异度高。
Objective To compare and analyze the results of rifampicin sensitivity detection by Xpert MTB/RIF and modified Roche proportional method.Methods A total of 246 patients with pulmonary tuberculosis who underwent Xpert MTB/RIF and modified Roche ratio test to detect rifampicin in the Fourth People's Hospital of Huai'an from July 2019 to April 2022 were selected as subjects.The coincidence rate of rifampicin sensitivity detection was compared between the two methods.Results There was no statistically significant difference in drug resistance rate of rifampicin detected by Xpert MTB/RIF and modified Roche proportional method(χ^(2)=1.15,P>0.05).Sensitivity was detected by proportional drug sensitivity test,and Xpert MTB/RIF resistance was found in 8 cases.Drug resistance was detected by proportional drug sensitivity test,and Xpert MTB/RIF sensitivity was 0 cases.Rifampicin resistance was detected in 36 of 246 samples by Xpert MTB/RIF.Among the five probes ProbeA,ProbeB,ProbeC,ProbeD and ProbeE designed by RRDR of Mycobacterium tuberculosis,63.9%of rifampicin resistance sites were detected by Xpert MTB/RIF system with ProbeE.Kappa=0.86 was used as the gold standard.Conclusion Xpert MTB/RIF assay for drug sensitivity of rifampicin has short time,high throughput,high sensitivity,high specificity and high accuracy,and has high clinical application value.
