摘要
目的探讨天麻钩藤饮加减治疗风阳上扰型急性脑梗死(AIS)的临床疗效。方法纳入风阳上扰型急性脑梗死患者120例,随机分为对照组与观察组,各60例。对照组单纯给予西医基础治疗,观察组除基础治疗外,加服天麻钩藤饮加减。总疗程共14 d。观察并记录2组治疗前后的NIHSS评分、BI评分、mRS评分及中医证候量化积分,疗程结束后评价2组神经功能缺损、日常自理能力及神经功能恢复情况,并综合评定临床疗效。结果治疗后,2组NIHSS评分、mRS评分、中医证候量化分级评分较治疗前均有降低,且观察组较对照组下降更为显著(P<0.05);2组BI评分较治疗前均有提高,且观察组较对照组提高更为显著(P<0.05);观察组总有效率(89.1%,49/55)明显高于对照组(75.0%,42/56)(P<0.05)。结论天麻钩藤饮加减治疗风阳上扰型急性脑梗死,能够有效改善神经缺损症状,提高生活质量,促进神经功能恢复,改善疾病预后,为中医辅助治疗急性脑梗死提供了有效的临床依据,值得临床推广。
Objective To explore the clinical efficacy of Tianma Gouteng Decoction in the treatment of acute cerebral infarction with the syndrome of wind-yang invading upward.Methods A total of 120 acute cerebral infarction patients with the syndrome of wind-yang invading upward admitted to our hospital from January 2019 to January 2022 were included and randomly divided into two groups,with 60 cases in each.The control group was only given a basic western medicine treatment,while the observation group was given Tianma Gouteng Decoction in addition to the basic treatment.The total course of treatment was both 14 days.The NIHSS scores,BI scores,mRS scores and TCM syndrome quantitative scores of the two groups were observed and recorded before and after treatment.After the treatment,the neurological function defi cit,daily self-care ability and neurological function recovery of the two groups were evaluated,and their clinical efficacy was comprehensively assessed.Results After treatment,the NIHSS scores,mRS scores,and quantitative classification scores of TCM syndromes in the two groups were all decreased compared with those before treatment,and the observation group decreased more significantly than the control group(P<0.05).The BI scores were improved compared with those before treatment,and the improvement in the observation group was more significant than that in the control group(P<0.05).After treatment,the total effective rate in the observation group(89.1%)was significantly higher than that in the control group(75.0%)(P<0.05).Conclusion Tianma Gouteng Decoction has a definite curative effect in the treatment of acute cerebral infarction with the syndrome of wind-yang invading upward,which can promote the recovery of nerve function,improve the symptoms of neurological deficit,improve the quality of life,and improve the prognosis of the disease,which provides an effective clinical basis support for the treatment of acute cerebral infarction with traditional Chinese medicine,worthy of clinical promotion.
