摘要
目的探讨穴位贴敷联合耳穴压豆对普外科术后使用止痛泵患者呕吐的效果。方法选择普外科术后呕吐患者128例,随机分为穴位贴敷组与联合耳穴压豆组,各64例。所有患者术后在常规处理基础上均使用止痛泵镇痛,穴位贴敷组采用穴位贴敷进行干预,联合耳穴压豆组在穴位贴敷组的基础上采用耳穴压豆进行干预,2组均干预48 h。比较2组术后6、12、24、48 h疼痛情况、呕吐发生分级情况及术前、术后48 h生活质量,统计2组干预期间并发症发生情况及干预完成后满意度。结果术后6、12、24、48 h,2组疼痛评分呈降低趋势,各时间点比较,差异具有统计学意义(P<0.05);术后6、12、24、48 h,联合耳穴压豆组疼痛评分低于穴位贴敷组(P<0.05)。术后12、24、48 h,联合耳穴压豆组呕吐一级患者占比(64.06%、75.00%、93.75%)高于穴位贴敷组(35.94%、51.56%、70.31%,P<0.05),术后12、24 h呕吐四级患者占比(14.06%、0.00%)低于穴位贴敷组(42.19%、14.06%,P<0.05)。术后48 h,2组收缩压(SBP)、舒张压(DBP)、心率(HR)均较治疗前升高(P<0.05);2组生活质量各项评分均升高,联合耳穴压豆组高于穴位贴敷组(P<0.05)。干预完成后,联合耳穴压豆组总满意率(92.19%)高于穴位贴敷组(75.00%)(P<0.05)。结论穴位贴敷联合耳穴压豆可有效缓解术后使用止痛泵患者呕吐症状,降低患者术后疼痛,提高患者生活质量,且不会对患者血流动力学指标产生影响,患者满意度更高,具有较好的临床效果。
Objective To study the effect of acupoint applications combined with auricular points pressing beans on preventing and treating vomiting in patients using analgesic pumps after general surgery.Methods A total of 128 patients after general surgery were selected as the research subjects,and divided into an acupoint application group(64 cases)and a combined auricular points pressing beans group(64 cases)according to the random number table.All the patients were treated with analgesic pumps for postoperative analgesia and a routine treatment after operation.On that basis,the acupoint application group was intervened with acupoint applications,while the combined auricular points pressing beans group was intervened with auricular points pressing beans on the basis of the acupoint application group,both for 48 hours.The pain,grade of vomiting at the 6th,12th,24th and 48th hour after operation,and the quality of life and hemodynamic indexes before and after 48 hours of intervention were compared between the two groups,and the complications during the intervention period and the satisfaction degree after the intervention were counted.Results At the 6th,12th,24th and 48th hour after operation,the scores of pain in the two groups showed a decreasing trend,and the differences were statistically significant(P<0.05).At the 6th,12th,24th and 48th hour after operation,the scores of pain of the combined auricular points pressing beans group were lower than those of the acupoint application group(P<0.05).At the 12th,24th and 48th hour after operation,the percentages of patients with Grade I vomiting in the combined auricular points pressing beans group(64.06%,75.00%,and 93.75%)were higher than those in the acupoint application group(35.94%,51.56%,and 70.31%)(P<0.05),and the percentages of patients with Grade IV vomiting at the 12th and 24th hour after operation(14.06%and 0.00%)were lower than those in the acupoint application group(42.19%and 14.06%)(P<0.05).At the 48th hour after operation,the levels of systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)of the two groups all increased(P<0.05),yet there was no significant difference between the two groups(P>0.05).At the 48th hour after operation,the scores of quality of life in the two groups increased,and the scores in the combined auricular points pressing beans group were higher than those in the acupoint application group(P<0.05).After the intervention,the total satisfaction rate of the combined auricular points pressing beans group(92.19%)was higher than that of the acupoint application group(75.00%)(P<0.05).Conclusion Acupoint application combined with auricular points pressing beans can effectively prevent and treat vomiting in patients using analgesic pumps after general surgery,reduce their postoperative pains,improve their quality of life,having no influence on their hemodynamic indexes,while increasing their satisfaction and having a good application effect.
