摘要
目的探讨高速涡轮机拔除下颌阻生第三磨牙致皮下气肿的原因,预防及处置策略。方法回顾性分析泰安市中心医院2019年2月至2022年2月就诊的13例高速涡轮机拔除下颌阻生第三磨牙致皮下气肿患者的临床资料,根据患者皮下气肿累及的范围及程度,划分为4个等级,分别予以不同处理。结果Ⅰ、Ⅱ级皮下气肿患者居家观察,口服抗生素,Ⅲ、Ⅳ级患者入院观察,对症处理及静滴抗生素,所有患者肿胀消退,恢复良好。结论应用高速涡轮机拔除下颌阻生第三磨牙致皮下气肿以保守治疗为主,但应密切观察,应用改良Ward手术切口能够降低皮下气肿的发生几率及等级。
Objective:To explore the causes,preventions and treatments of subcutaneous emphysema following extraction of mandibular third impact molars with high-speed handpiece.Methods:Thirteen cases of subcutaneous emphysema following extraction of mandibular third impact molars with high-speed handpiece were reviewed and analyzed from February 2019 to February 2022 in Taian City Central Hospital.According to the extent of subcutaneous emphysema,all patients were divided into four grades with different treatments individually.Results:Patients of grade I andⅡsubcutaneous emphysema were observed at home with oral antibiotics,while patients of gradeⅢandⅣsubcutaneous emphysema were admitted for observation,symptomatic treatments and intravenous antibiotics.The swellings resolved during 3 to 9 days and all patients recovered well.Conclusion:Closely clinical observation and conservative treatments should be applied to subcutaneous emphysema following mandibular third impact molars extraction.The application of modified surgical incision can reduce the incidence of subcutaneous emphysema and the grade of emphysema.
