摘要
目的探讨微型钢板内固定治疗掌指关节周围骨折的效果。方法回顾性分析2018年8月-2020年12月收治的42例掌指关节周围骨折患者临床资料,按照随机数字表法分组,对照组21例,给予克氏针内固定治疗;观察组21例,给予微型钢板内固定治疗。观察两组手指主动屈曲度情况、治疗情况、掌指关节活动与疼痛、并发症。结果观察组手指总主动屈曲量表(TAFS)评价优良率为85.71%,对照组TAFS优良率为57.14%,观察组TAFS优良率显著高于对照组(P<0.05);观察组手术时间长于对照组,骨折愈合时间、功能训练时间、重返工作岗位时间均短于对照组(P<0.05);观察组掌指关节活动度大于对照组,VAS量表评分小于对照组(P<0.05);观察组发生骨折畸形愈合、内固定物松脱、关节僵硬及肌腱粘连并发症的发生率为9.52%,对照组为42.86%,观察组并发症发生率明显低于对照组(P<0.05)。结论微型钢板内固定治疗掌指关节周围骨折可显著改善手指主动屈曲度及掌指关节活动度,提高治疗效果,且能显著降低疼痛情况及并发症的发生率。
Objective To investigate the effect of mini plate internal fixation in the treatment of fractures around metacarpophalangeal joint.Methods Forty-two patients with fractures around metacarpophalangeal joint in our hospital from August 2018 to December 2020 were divided into two groups according to the random number table method.21 patients in the control group were treated with Kirschner wire internal fixation,and 21 patients in the observation group were treated with micro plate internal fixation.The active flexion degree,treatment,metacarpophalangeal joint activity,pain and complications of the two groups were observed.Results The excellent and good rate of TAFS in the observation group was 85.71%,and that in the control group was 57.14%.The excellent and good rate of TAFS in the observation group was significantly higher than that in the control group(P<0.05);The operation time in the observation group was longer than that in the control group,and the fracture healing time,functional exercise time and return to work time in the observation group were shorter than those in the control group(P<0.05);The range of metacarpophalangeal joint motion in the observation group was greater than that in the control group,and the score of vas scale was lower than that in the control group(P<0.05);The incidence of complications of fracture healing deformity,internal fixation loosening,joint stiffness and tendon adhesion in the observation group was 9.52%,and that in the control group was 42.86%.The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Mini plate internal fixation in the treatment of fractures around metacarpophalangeal joint can significantly improve the active flexion of fingers and the range of motion of metacarpophalangeal joint,improve the treatment effect,and significantly reduce the incidence of pain and complications.
