摘要
目的:研究自拟健脾益气汤联合甲地孕酮对结肠癌食欲不振-恶病质综合征的临床疗效。方法:选取2021年6月-2022年6月收治的60例结肠癌患者为研究对象,按照随机抽样法分为对照组(n=30)和研究组(n=30)。对照组予以甲地孕酮治疗,研究组在对照组基础上予以自拟健脾益气汤治疗。比较治疗前后两组患者的中医证候积分、食欲不振-恶病质综合征功能性评估量表(FAACT)评分、行为状态评分(KPS)、肿瘤标志物[血清糖类抗原72-4(CA72-4)]水平、细胞因子[肿瘤坏死因子-α(TNF-α)、IL-6]水平,比较治疗后两组患者的体重变化情况、食欲改善情况,并统计两组患者的不良反应发生情况。结果:治疗后,研究组患者的体重增加、食欲改善、FAACT及KPS评分均高于对照组,均差异有统计学意义(P<0.05),中医证候评分、CA72-4、TNF-α、IL-6水平、不良反应发生率均低于对照组,均差异有统计学意义(P<0.05)。结论:自拟健脾益气汤联合甲地孕酮可改善结肠癌患者的食欲,增加体重,增强行为能力,提高生命质量,降低炎症反应,改善中医证候积分,安全有效。
Objective:To study the clinical effect of self-made Jianpi Yiqi Decoction combined with megestrol acetate on colon cancer with anorexia-cachexia syndrome.Methods:Sixty patients with colon cancer admitted to our hospital from June 2021to June 2022were selected and divided into control group(n=30)and study group(n=30)according to the random number table method.The control group was treated with megestrol acetate,and the study group was treated with self-made Jianpi Yiqi Decoction on the basis of the control group.The scores of Traditional Chinese Medicine(TCM)symptoms,functional assessment of appetite and cachexia therapy syndrome(FAACT)scores,Karnofsky performance scores(KPS),tumor markers[serum carbohydrate antigen 72-4(CA72-4)]levels,and cytokines[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]levels were compared between the two groups before and after treatment.Changes in body weight and appetite improvement were compared between the two groups after treatment,and the occurrence of adverse reactions in the two groups.Results:After treatment,the weight gain,appetite improvement,FAACT and KPS scores in the study group were higher than those in the control group,and the TCM syndrome score,CA72-4,TNF-α,IL-6levels and incidence of adverse reactions were lower than those in the control group(P<0.05).Conclusion:Jianpi Yiqi Decoction combined with megestrol acetate can improve appetite in patients with colon cancer,increase weight,enhance capacity,improve quality of life,reduce inflammation,improve TCM syndrome,safe and effective.
