妊娠期接种新型冠状病毒疫苗的安全性评价 被引量:3

摘要 目的:通过分析致畸率、妊娠不良结局,探讨妊娠期接种新型冠状病毒疫苗(简称新冠疫苗)的安全性。方法:收集2020年10月至2022年3月在香港大学深圳医院产前诊断中心进行遗传咨询的妊娠期接种新冠疫苗孕妇共67例,进行孕期全周期的追踪随访,分析超声结构筛查结果和妊娠结局。选择本院同期未接种新冠疫苗的9250例孕妇作为对照,比较两组的致畸率、自然流产率、早产率和出生低体重率。结果:新冠疫苗接种组中自然流产8例(11.9%,8/67),59例顺利妊娠至分娩。早/中孕期超声筛查异常5例(8.5%,5/59),其中胎儿结构异常1例(1.7%,1/59)、超声软指标异常4例(6.8%,4/59),与未接种组比较,差异无统计学意义(P>0.05)。新冠疫苗接种组发生早产10例(16.9%,10/59),早产率高于未接种组(P<0.05);出生低体重儿3例(5.1%,3/59),与未接种组无统计学差异。结论:妊娠期接种新冠疫苗未增加致畸或出生缺陷风险,自然流产率、出生低体重率与未接种者无差异。 Objective:By analyzing the rate of fetal malformation and adverse pregnancy outcomes,the safety of COVID-19 vaccination during pregnancy was discussed.Method:From October 2020 to March 2022,a total of 67 pregnant women with COVID-19 vaccination during pregnancy were recruited at the prenatal diagnosis center of the University of Hong Kong-Shenzhen Hospital.9250 women were recruited at obstetric clinic as control.All pregnant women were followed-up at the University of Hong Kong-Shenzhen Hospital during the whole pregnancy.Results of ultrasonic structural screening,rate of fetal malformation,adverse pregnancy out-come,miscarriage rate,premature birth rate,birth weight and pregnancy outcomes were compared.Results:In the COVID-19 vaccination group,there were 8 cases of miscarriage(11.9%,8/67),and the remaining 59 cases were followed to delivery.There were 5 cases(8.5%,5/59)of abnormal ultrasonic screening in early/mid-pregnancy,including 1 case(1.7%,1/59)of abnormal fetal morphology and 4 cases(6.8%,4/59)of abnormal ultrasonic soft markers.There was no statistically significant difference between vaccinated and unvaccinated group(P>0.05).Premature delivery occurred in 10 cases(16.9%,10/59)in the COVID-19 vaccination group,and the premature delivery rate was significantly higher than that in the unvaccinated group(P<0.05).There were 3 cases of low-birth-weight infants(5.1%,3/59),with no significant difference compared with the unvaccinated group.Conclusion:Vaccination with COVID-19 during pregnancy does not increase the risk of fetal malformation or birth defects,and there is no significant difference in rates of miscarriage and low birth weight between vaccinated and unvaccinated pregnant women.
