摘要 目的:探讨神经介入术联合静脉溶栓治疗缺血性脑血管疾病的临床效果与安全性。方法:选取2020年4月—2021年11月山东省菏泽市第三人民医院收治的71例缺血性脑血管疾病患者为研究对象,根据入院单双日的顺序分为试验组与对照组。对照组给予药物联合静脉溶栓治疗,试验组行神经介入联合静脉溶栓治疗。对比两组临床疗效、血管再通率、美国国立卫生研究院卒中量表(NIHSS)评分、血流动力学及不良反应发生情况。结果:试验组总有效率高于对照组,差异有统计学意义(P=0.005)。试验组血管再通率高于对照组,差异有统计学意义(P=0.002)。治疗前,两组NIHSS评分比较,差异无统计学意义(P=0.663);治疗后,两组NIHSS评分均低于治疗前,且试验组低于对照组,差异有统计学意义(P=0.000)。治疗前,两组椎动脉、基底动脉、颈内动脉及颈总动脉收缩期峰流速比较,差异无统计学意义(P>0.05);治疗后,试验组椎动脉、基底动脉、颈内动脉及颈总动脉收缩期峰流速均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P=0.732)。结论:神经介入术联合静脉溶栓治疗缺血性脑血管疾病效果良好,值得应用。 Objective:To explore the clinical effect and safety of nerve intervention combined with intravenous thrombolysis in the treatment of ischemic cerebrovascular diseases.Methods:A total of 71 patients with ischemic cerebrovascular disease admitted to the Third People's Hospital of Heze in Shandong Province from April 2020 to November 2021 were selected as the study subjects.Patients were divided into experimental group and control group according to the order of admission day(odd number and even number).The control group received drugs combined with intravenous thrombolysis treatment,and the experimental group received nerve intervention combined with intravenous thrombolysis treatment.The clinical efficacy,vascular recanalization rate,National Institutes of Health Stroke Scale(NIHSS)score,hemodynamics and incidence of adverse reactions between the two groups were compared.Results:The total effective rate in the experimental group was higher than that in the control group,and the difference was statistically significant(P=0.005).The vascular recanalization rate in the experimental group was higher than that in the control group,and the difference was statistically significant(P=0.002).Before treatment,there was no significant difference in the NIHSS scores between the two groups(P=0.663).After treatment,the NIHSS scores in two groups were lower than those before treatment,and the experimental group was lower than the control group,and the difference was statistically significant(P=0.000).Before treatment,there was no significant difference in the peak systolic flow velocity of vertebral artery,basilar artery,internal carotid artery and common carotid artery between two groups(P>0.05).After treatment,the peak systolic flow velocity of vertebral artery,basilar artery,internal carotid artery and common carotid artery in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P=0.732).Conclusion:Nerve interventional combined with intravenous thrombolysis is effective in the treatment of ischemic cerebrovascular diseases,which is worthy of application.
