摘要
目的观察自拟疏肝散结汤联合甲巯咪唑片治疗气滞痰凝证甲状腺功能亢进症的临床疗效。方法选取2019年1月—2020年7月期间北京市怀柔区中医医院收治的甲状腺功能亢进症患者86例,采用随机数字表法分为治疗组和对照组,每组各43例。对照组给予甲巯咪唑片治疗,治疗组在对照组基础上给予自拟疏肝散结汤治疗。治疗28 d后,观察比较两组患者临床疗效、不良反应,治疗前后中医证候积分、甲状腺激素[甲状腺激素总三碘甲状腺原氨酸(Total-triiodothyronine,TT3)、总甲状腺素(Total Thyroxine,TT4)、游离三碘甲状腺原氨酸(Free triiodothyronine,FT3)、游离甲状腺素(Free Thyroxine,FT4)、血清促甲状腺素(Thyroid-stimulatinghormone,TSH)]水平。结果治疗后治疗组总有效率95.35%(41/43)明显高于对照组总有效率76.74%(33/43),差异有统计学意义(P<0.05)。治疗后两组患者中医证候评分均较治疗前降低,差异有统计学意义(P<0.05);且治疗组中医证候评分较对照组明显降低,差异有统计学意义(P<0.05)。治疗后两组患者TT3、TT4、FT3、TF4水平均较治疗前降低,TSH水平均较治疗前升高,差异有统计学意义(P<0.05);且治疗组TT3、TT4、FT3、TF4水平均较对照组降低,TSH水平较对照组升高,差异有统计学意义(P<0.05)。治疗期间,治疗组不良反应总发生率6.98%(3/43)明显低于对照组44.19%(19/43),差异有统计学意义(P<0.05)。结论自拟疏肝散结汤联合甲巯咪唑片治疗甲状腺功能亢进症(气滞痰凝证)确有良好疗效,且安全性较高。
Objective To observe the clinical effect of self-made Shugan Sanjie Decoction combined with Methimazole Tablets on hyperthyroidism(qi stagnation and phlegm coagulation syndrome).Methods Eighty-six cases of hyperthyroidism(qi stagnation and phlegm coagulation syndrome)admitted to Beijing Huairou Hospital of Traditional Chinese Medicine from January 2019 to July 2020 were included and divided into a treatment group(n=43)and a control group(n=43)according to random number table method.The control group was treated with Methimazole Tablets,while the treatment group was given self-made Shugan Sanjie Decoction on the basis of the control group.After 28 days of treatment,the treatment efficacy and adverse reaction of the two groups,as well as the traditional Chinese medicine(TCM)syndrome scores and the levels of thyroid hormones[total triiodothyronine(TT3),total thyroxine(TT4),free triiodothyronine(FT3),free thyronine(FT4),and thyroid-stimulating hormone(TSH)]before and after treatment were observed and compared.Results The total effective rate was 95.35%(41/43)in the treatment group and 76.74%(33/43)in the control group,and the difference was significant(P<0.05).After treatment,the TCM syndrome scores in both groups were lower than those before treatment(P<0.05),and the treatment group had lower scores than the control group(P<0.05).There was a decrease in TT3,TT4,FT3,and FT4 while an increase in TSH in the two groups after treatment compared with those before treatment(P<0.05),and the treatment group was better than the control group in these indexes(P<0.05).In terms of adverse reactions during treatment,the incidence rate in the treatment group was 6.98%(3/43),lower than 44.19%(19/43)in the control group(P<0.05).Conclusion Self-made Shugan Sanjie Decoction combined with Methimazole Tablets has an excellent curative effect on hyperthyroidism(qi stagnation and phlegm coagulation syndrome)with good safety.
