摘要
目的鉴定脐带血污染的细菌种类,探讨细菌来源,旨在为操作脐带血的人员提供指导,最大限度减少因人为原因造成的脐带血污染废弃。方法抽取2021年6月16日—8月31日由山东省脐带血造血干细胞库收集的脐带血样本21399例,采用全自动血培养仪,进行需氧菌(含兼性厌氧菌)和厌氧菌培养,并对血培养阳性标本送检至第三方检测机构进行细菌鉴定。结果21399例脐带血样本经培养后阳性样本共计292例,阳性率为1.36%,需氧阳性51例,占阳性比例为17.47%,厌氧阳性134例,占阳性比例为45.89%,双阳107例,占阳性比例为36.64%。剖宫产脐带血样本阳性40例,占阳性比例为13.70%,自然分娩脐带血样本阳性252例,占阳性比例为86.30%。292例阳性样本进行细菌鉴定,分离出405株细菌,其中需氧菌菌株162株,占总菌株的40%,厌氧菌菌株243株,占总菌株的60%,其中单形拟杆菌、产气柯林斯菌、加氏乳杆菌只在自然分娩中鉴定出。结论厌氧菌(含兼性厌氧菌)是导致脐带血污染最主要的菌株,自然分娩的方式脐带血样本污染率较高,采集脐带血的过程中严格执行无菌操作会降低脐带血的细菌污染率。
Objective To identify the bacterial species of umbilical cord blood pollution and explore the source of bacteria,so as to provide guidance for the treatment of umbilical cord blood and minimize the pollution and waste of umbilical cord blood caused by human intervention.Methods A total of 21399 umbilical cord blood samples from Shandong Province Cord Blood Hematopoietic Stem Cell Bank from June 16 to August 31,2021 were collected.Aerobic bacteria(including facultative anaerobic bacteria)and anaerobic bacteria were cultured by automatic blood culture apparatus,and the positive samples were sent to a third-party testing institution for bacterial identification.Results Among 21399 umbilical cord blood samples,there were 292 positive samples,the positive rate was 1.36%,with 51 aerobic positive samples,accounting for 17.47%,134 anaerobic positive samples,accounting for 45.89%,and 107 double positive samples,accounting for 36.64%.40 positive samples were in cesarean section(13.70%)and 252 positive samples were in natural delivery(86.30%).292 positive samples were identified and 405 strains of bacteria were isolated,including 162 strains of aerobic bacteria,accounting for 40%of the total strains,and 243 strains of anaerobic bacteria,accounting for 60%of the total strains.Among them,Bacteroides simplex,Collins aerogenes and Lactobacillus gasseri were only identified in natural childbirth.Conclusion Anaerobic bacteria(including facultative anaerobic bacteria)are the most important strains that cause cord blood pollution.Natural delivery leads to a high contamination rate of cord blood samples.Strict aseptic operation during cord blood collection will reduce the bacterial contamination rate of cord blood.
