不同易感人群尿路感染病原菌分布及耐药性分析 被引量:2

Analysis of the Distribution and Drug Resistance of Pathogenic Bacteria of Urinary Tract Infection in Different Sus⁃ceptible Groups

摘要 目的:通过对不同易感人群尿路感染病原菌分布及耐药性的分析,为临床尿路感染的治疗提供参考。方法:根据基础疾病的不同将易感人群分为结石组、糖尿病组、脑卒中组以及全体住院组,对分离的病原菌采用WHONET2018版、SPSS 25.0版软件进行相应的统计分析。结果:糖尿病组以女性患者为主;结石组以41~70岁为主,脑卒中组则以61~90岁为主;糖尿病组大肠埃希菌占比最高,脑卒中组肺炎克雷伯菌检出相对较高,与住院组比较差异均有统计学意义(χ2=12.570、9.491,P<0.05),结石组白假丝酵母菌占比较高;三组易感人群大肠埃希菌耐药率从高到底依次为结石组、脑卒中组、糖尿病组,结石组超广谱β-内酰胺酶(ESBLs)阳性率最高,脑卒中组耐药率与住院组最为接近,并出现耐碳青霉烯的大肠埃希菌和肺炎克雷伯菌。结论:不同易感人群间尿路感染病原菌分布和耐药性存在一定差异,临床应尽可能地早期送检尿培养并根据病原菌药物敏感性及时进行抗生素调整,同时加强尿路置管的护理,减少和防止耐药菌的产生。 Objective:By analyzing the distribution and drug resistance of pathogenic bacteria of urinary tract infections in dif⁃ferent susceptible groups,to provide reference for the treatment of clinical urinary tract infections.Methods:The susceptible popu⁃lation was divided into a stone group,a diabetes group,a stroke group,and an all hospitalization group according to the underlying disease.WHONET 2018,SPSS 25.0 software was used to perform the corresponding statistical analysis of the isolated pathogenic bacteria.Results:The diabetic group was dominated by female patients,the stone group was dominated by 41-70 years old,and the stroke group was dominated by 61-90 years old.The diabetic group had the highest percentage of Escherichia coli and the stroke group had a relatively high detection of Klebsiella pneumoniae,with statistically significant differences compared with the hospitalization group(χ2=12.570,9.491,P<0.05).The percentage of Pseudomonas albicans was higher in the stone group.The three susceptible groups of E.coli resistance rates were,in descending order,the stone group,the stroke group,and the diabetes group.The stone group had the highest rate of ultra broad-spectrum B-lactamases(ESBLs)positivity,and the stroke group had the closest rate of resistance to the hospitalization group,with carbapenem-resistant Escherichia coli and Klebsiella pneumoniae.Con⁃clusion:The distribution and drug resistance of pathogenic bacteria in urinary tract infections vary among different susceptible populations,so clinics should send urine cultures as early as possible and make timely antibiotic adjustments according to the drug sensitivity of pathogenic bacteria,while strengthening the care of urinary tract placement to reduce and prevent the production of drug-resistant bacteria.
