摘要
目的:观察健康行动过程取向模型的菜单式护理对冠心病心绞痛患者医嘱遵从性及心血管不良事件的影响。方法:选择2018年3月—2020年3月河南科技大学第一附属医院收治的62例冠心病心绞痛患者作为研究对象,随机分为常规护理组和菜单式护理组,每组各31例。常规护理组患者采用常规的护理方式,菜单式护理组患者在常规护理的基础上进行健康行动过程取向模型的菜单式护理。护理后,评估两组患者的护理满意度、遵医行为和自我效能;护理前后,评估两组患者对冠心病的认知度,记录护理过程中心血管不良事件发生情况。结果:护理后,菜单式护理组患者的环境护理、基础护理、治疗护理、心理护理各项护理满意度明显高于常规护理组,差异有统计学意义(t=2.650,2.189,2.099,2.658,P<0.05)。护理后,菜单式护理组和常规护理组患者对冠心病疾病病因、临床症状、治疗方法、预后转归的认知度明显高于护理前,差异有统计学意义(t=2.456、2.064、2.431、2.332,P<0.05)。菜单式护理组患者对冠心病相关知识的认知度明显高于常规护理组,差异有统计学意义(t=2.456,2.064,2.431,2.331,P<0.05)。护理后,菜单式护理组患者的饮食控制、作息规律、戒烟戒酒、疾病认知和治疗态度各项遵医行为评分高于常规护理组,差异有统计学意义(t=3.364、2.899、2.316、2.595,P<0.05)。护理后,菜单式护理组自我效能高于常规护理组,差异有统计学意义(t=7.728,P<0.05);菜单式护理组不良事件发生率明显低于常规护理组,差异有统计学意义(χ2=4.292,P<0.05)。结论:健康行动过程取向模型的菜单式护理能有效提高冠心病心绞痛患者遵医性和自我效能,有效降低心血管不良事件发生率。
Objective:To observe the effect of menu-based care with a health action process orientation model on medical compliance and adverse cardiovascular events in patients with angina pectoris in coronary artery disease.Methods:62 patients with coronary angina admitted to the hospital from March 2018 to March 2020 were selected as the study population and randomly divided into conventional care group and menu-based care group,with 31 cases each.The conventional care group used conventional care,and the menu-based care group performed menu-based care care based on the health action process orientation model based on the care of the conventional care group.After care,patients in both groups were assessed for satisfaction with care,compliance behavior and self-efficacy.Before and after care,the patients’awareness of coronary heart disease was assessed in both groups,and the occurrence of cardiovascular adverse events during care was recorded.Results:After the care,the satisfaction of each care of environmental care,basic care,therapeutic care,and psychological care was significantly higher in the menu care group than in the conventional care group,and the difference was statistically significant(t=2.650,2.189,2.099,2.658,P<0.05).After care,the awareness of the causes,clinical symptoms,treatments,and prognostic regression of coronary heart disease in the menu-based care group and the conventional care group was significantly higher than that before care,with statistically significant differences(t=2.456,2.064,2.431,2.332,P<0.05).The awareness of knowledge related to coronary heart disease was significantly higher in the menu-based care group than in the conventional care group,with statistically significant differences(t=2.456,2.064,2.431,2.331,P<0.05).After care,the compliance behavior scores of diet control,work and rest routine,smoking and alcohol cessation,disease awareness and treatment attitude were higher in the menu-based care group than in the conventional care group,and the differences were statistically significant(t=3.364,2.899,2.316,2.595,P<0.05).The self-efficacy of the post-care menu care group was higher than that of the usual care group,and the difference was statistically significant(t=7.728,P<0.05).The incidence of adverse events was significantly lower in the menu-based care group than in the usual care group,with a statistically significant difference(χ2=4.292,P<0.05).Conclusion:The Health Action Process Orientation Model of menu-based care is effective in improving compliance and self-efficacy in patients with coronary angina and effectively reducing the incidence of adverse cardiovascular events.
