摘要
目的 探讨更优的CTU(泌尿系CT造影)扫描方法在膀胱占位患者中的临床应用价值.方法 选择2021年3月至10月行泌尿系CTU检查中膀胱病变的患者67例,随机选取36例行常规CTU扫描,余36例行CTU优化扫描方案(其中5例是已行常规CTU扫描的同一患者).观察并分析2组图像,测量肾盂、膀胱充盈后最大截面处上下端的CT值,竖脊肌CT值及竖脊肌的SD值,并计算得出肾盂及膀胱充盈后最大截面处上下端的SNR、CNR值,对两组定量参数进行分析.结果 改进前后输尿管连续性评分差异无统计学意义(P>0.05),两组间肾盂SNR、CNR差异无统计学意义(P>0.05),膀胱充盈最大截面下端两组间SNR、CNR差异无统计学意义(P>0.05).膀胱充盈最大截面上端两组间SNR、CNR差异有统计学意义(P>0.05),两组间膀胱显示评分差异有统计学意义(P>0.05),且优化后的扫描方案膀胱内不存在分层现象,可以更好显示膀胱病灶的部位、形态及周围结构情况,为诊断提供更准确的图像信息.结论 优化后的CTU扫描方案在膀胱占位患者中具有更好的临床应用价值.
Objective To explore the clinical application value of better computed tomography urography(CTU)scanning method in patients with bladder mass.Methods A total of 67 patients with bladder lesions who underwent urinary CTU examination in our hospital from March 2021 to October 2021 were collected.36 cases were selected randomly for routine CTU scan and the remaining 36 cases were selected for CTU optimized scan(5 of them were the same patient who had undergone routine CTU scans).The images of the two groups were observed and analyzed.The CT value of renal pelvis and the CT value of the upper and lower ends of the maximum section of bladder flling,the CT value and the SD value of erector spinal muscle were measured.The SNR and CNR of the renal pelvis and the upper and lower ends of the maximum section of bladder flling according to the formula were calculated.Statistical analysis was conducted on quantitative parameters of the two groups,P<0.05 indicated a statistical difference.Results There was no statistical difference in the ureteral continuity scores before and after the improvement(Z=-0.716,P=0.474),the SNR and CNR of renal pelvis(Z=-0.534,-0.770,P=0.593,0.441)and the SNR and CNR of the lower end of the maximum section of bladder flling(Z=-1.744,-1.602,P=0.081,0.109)also had no significant difference between the two groups.The SNR and CNR of the upper end of the maximum section of bladder flling(Z=-5.232,-5.216,P=0.000)and the score of bladder display(Z=-7.161,P<0.001)between the two groups had significant diference.Moreover,the optimized scanning scheme did not have a stratification phenomenon in the bladder,which could better display the location,morphology and surounding structure of the bladder lesions,and provide moreaccurate imageinformationfordiagnosis.Conclusion The optimized CTU scanning protocol has better clinical utility in patients with bladder mass.
