摘要
目的:研究食管癌^(18)F-氟代脱氧葡萄糖(^(18)F-fluorodeoxyglucose,^(18)F-FDG)PET/CT显像代谢参数。总病灶糖酵解值(total lesion glycolysis,TLG)、肿瘤代谢体积(metabolic tumor volume,MTV)、最大标准摄取值(maximum standardized uptake value,SUV_(max))的临床价值。方法:选取2020年7月—2022年8月于常州市肿瘤医院核医学科行^(18)F-FDG PET/CT显像的54例初诊患者资料,所有患者最后均经内镜或手术病理学检查确诊为食管癌。计算代谢参数TLG、MTV、SUV_(max),并回顾性研究三者分别与病灶最大横径、长度和最大横截面积的相关性,长度不同的原发病灶组间3个代谢参数各自的差异,以及食管癌原发病灶这3个代谢参数分别与淋巴结和(或)远处转移的相关性。结果:54例食管癌患者的病灶TLG、MTV、SUV_(max)分别与病灶的最大横径、长度和最大横截面积相关(P<0.05)。长度<3cm与3~5cm病灶SUV_(max)之间差异有统计学意义(t=2.108,P=0.049),<3 cm与>5cm病灶SUV_(max)之间差异有统计学意义(t=2.510,P=0.017),但3~5cm与>5cm病灶SUV_(max)之间差异无统计学意义(t=1.788,P=0.080)。长度<3cm、3~5cm、>5cm病灶三组间MTV两两比较,差异均有统计学意义(t值分别为3.227、3.250、2.342,P值分别为0.004、0.002、0.025)。长度<3 cm、3~5cm、>5 cm病灶三组间TLG两两比较,差异均有统计学意义(t值分别为2.946、3.378、2.416,P值分别为0.008、0.001、0.021)。无转移、仅淋巴结转移和远处转移三组间SUV_(max)、MTV和TLG差异均无统计学意义(F值分别为0.952、0.407、0.424,P值分别为0.393、0.667、0.657)。结论:食管癌^(18)F-FDG PET/CT显像代谢参数TLG、MTV、SUV_(max)与原发病灶的大小高度相关,但三者均无法预测是否有淋巴结和(或)远处转移。
Objective To investigate the clinical significance of total lesion glycolysis(TLG),metabolic tumor volume(MTV)and maximum standardized uptake value(SUV_(max))of ^(18)F-fluorodeoxyglucose(FDG)PET/CT imaging in esophageal cancer.Methods The data of 54 newly diagnosed patients who underwent ^(18)F-FDG PET/CT imaging in the Department of Nuclear Medicine,Changzhou Tumor Hospital from July 2020 to August 2022 were selected.All patients were finally diagnosed as esophageal cancer by endoscopy or surgical pathology.The relationship between TLG,MTV,SUV_(max) and the size of tumor(maximum diameter,length and maximum cross sectional area,respectively),the difference inS UVmax,MTV and TLG between length,and the relationship between SUV_(max),MTV and TLG of the primary esophageal carcinoma lesions and metastasis were analyzed.Results The correlation of SUV_(max) with maximum diameter,length and maximum cross sectional area was statistically significant respectively,The correlations of MTV and TLG with maximum diameter,length and maximum cross sectional area were also statistically significant respectively(P<0.05).The differences in SUV_(max) between groups<3 cm and 3~5 cm and between groups<3 cm and>5 cm were statistically significant(t=2.108,P=0.049;t=2.510,P=0.017),but the difference between groups 3~5 cm and>5 cm was not statistical significant(t=1.788,P=0.080).The differences in MTV between groups<3 cm and 3~5 cm,between groups 3~5 cm and>5 cm,and between groups<3 cm and>5 cm were statistically significant(t=3.227,3.250,2.342;P=0.004,0.002,0.025).The differences in TLG between groups<3 cm and 3~5 cm,between groups 3~5 cm and>5 cm,and between groups<3 cm and>5 cm were statistically significant(t=2.946,3.378,2.416;P=0.008,0.001,0.021).SUV_(max),MTV and TLG of the primary lesions were not correlated with metastasis(F=0.952,0.407,0.424;P=0.393,0.667,0.657).Conclusion SUV_(max),MTV and TLG of ^(18)F-FDG PET/CT imaging have important values in evaluating the size of esophageal carcinoma,but they are not related to lymph node metastasis or distant metastasis.
