壮医针刺对神经病理性疼痛大鼠的镇痛作用及其机制研究

摘要 目的探讨通过壮医针刺调节miR-429、IKKβ的表达对神经病理性疼痛大鼠产生镇痛作用的相关机制。方法将50只SD大鼠随机分为5组:正常组、假手术组、模型组、非经非穴组、壮医针刺组,每组10只。除外正常组和假手术组,余下3组均采用结扎L5脊神经的方法建立模拟神经病理性疼痛的模型(SNL模型)。在术前(0天)、手术后第1天、手术后第14天检测各组大鼠机械性刺激缩足阈值(PWMT)。手术造模后第1天,检测行为学指标,随后对非经非穴组、壮医针刺组开始针刺干预,连续14天,其余3组不进行干预,至术后第14天取大鼠L4~6脊髓背角组织行PCR检测miR-429表达量,Western Blot检测IKKβ蛋白含量,行ELISA检测大鼠血清白细胞介素1β(IL-1β)含量。结果术前各组大鼠的PWMT差异无统计学意义(P>0.05);术后第1天模型组、非经非穴组、壮医针刺组PWMT显著降低(P<0.01),并且高于正常组、假手术组(P<0.01),模型组、非经非穴组、壮医针刺组之间的PWMT差异无统计学意义(P>0.05);术后第14天壮医针刺组PWMT高于模型组和非经非穴组(P<0.01)。术后第14天,正常组大鼠脊髓背角组织中miR-429表达处于高水平,并且高于模型组、非经非穴组、壮医针刺组大鼠脊髓背角组织中miR-429表达水平(P<0.01),与模型组、非经非穴组相比,壮医针刺组miR-429表达水平升高(P<0.05)。术后第14天,正常组大鼠IKKβ、IL-1β表达处于低水平,与正常组相比,模型组脊髓背角组织IKKβ、血清IL-1β表达水平升高(P<0.01),与模型组、非经非穴组相比,壮医针刺组可下调大鼠IKKβ、IL-1β表达水平(P<0.05或0.01)。结论壮医针刺可改善SNL模型大鼠疼痛情况,其作用机制与上调脊髓背角组织miR-429表达,抑制IKKβ蛋白的含量,减少IL-1β的产生,从而减轻上述过程所介导的免疫炎症刺激而引起疼痛的效应有关。 Objective To explore the relevant mechanisms of analgesic effects on neuropathic pain of rats through regulating the expressions of miR-429 and IKKβby Zhuang acupuncture.Methods 50 SD rats were randomly divided into normal group,sham operation group,model group,non-point group,and Zhuang acupuncture group,with 10 rats in each group.Except for the normal group and the sham group,the remaining three groups were all established the spinal nerve ligation(SNL)model which simulated neuropathic pain with the method of ligating the L5 spinal nerve.Paw withdrawal mechanical threshold(PWMT)of rats in each group was measured before operation,on the 1 st day and the 14 th day after operation.On the first day after surgical modeling,behavioral indicators were tested,and then acupuncture intervention was started in the non-point group and the Zhuang acupuncture group for 14 consecutive days.The other 3 groups did not receive any intervention.On the 14 th day after surgery,the L4-6 dorsal horn of the spinal cord of the rats was taken for PCR to detect the expression of miR-429,and Western Blot was used to detect the protein content of IKKβ,and ELISA was used to detect the serum IL-1βcontent of rats.Results There was no statistically significant difference in PWMT of rats in each group before operation(P>0.05);on the 1 st day after operation,the PWMT of the model group,the non-point group and the Zhuang acupunture group significantly decreased(P<0.01),and those of the above-mentioned three groups were higher than those of the control group and the sham group(P<0.01).There was no statistically significant difference in PWMT of the model group,the non-point group and the Zhuang acupunture group(P>0.05);on the 14 th day after operation,the PWMT in the Zhuang acupuncture group was higher than that in the model group and the non-acupoint group(P<0.01).On the 14 th day after operation,the expression of miR-429 in the spinal dorsal horn tissue of rats in the normal group was at a high level,and was higher than that of the model group,the non-acupoint group,and the Zhuang acupuncture group(P<0.01),and compared with the model group and the non-acupoint group,the expression level of miR-429 in the Zhuang acupuncture group was higher(P<0.05).On the 14 th day after surgery,IKKβand IL-1βexpressions in the normal group were low,and compared with the normal group,the expression levels of IKKβand serum IL-1βincreased in the dorsal horn tissue in the model group(P<0.01),and compared with the model group and the non-acupoint group,the Zhuang acupuncture group could down-regulate the expression levels of IKKβand IL-1βin rats(P<0.05 or 0.001).Conclusion Zhuang acupunture can alleviate the hyperalgesia of SNL model rats,and its mechanism is related to up-regulate the expression of miR-429 in the spinal dorsal horn tissue and inhibit the protein content of IKKβ,reduce the production of IL-1β,and thus alleviating the effects of immune inflammatory stimulation mediated by the above-mentioned processes on pain.
