摘要
目的 探讨肺炎克雷伯菌和大肠埃希菌的耐药率与抗菌药物用药频度的相关性,为临床合理使用和管控抗菌药物提供参考。方法 回顾性调查某医院2016年1月—2020年12月住院患者分离大肠埃希菌、肺炎克雷伯菌的耐药率和抗菌药物使用量,对抗菌药物用药频度(DDDs)与细菌耐药率进行Pearson相关性分析。结果 肺炎克雷伯菌对哌拉西林/他唑巴坦、头孢曲松和美罗培南的耐药率与这些药物的DDDs呈极强正相关关系(r=0.931、P=0.021,r=0.880、P=0.049,r=0.921、P=0.027);大肠埃希菌对阿米卡星和美罗培南的耐药率与两种药物DDDs呈极强正相关(r=0.988,P=0.002;r=0.951,P=0.013)。结论 大肠埃希菌、肺炎克雷伯菌的耐药率与部分抗菌药物DDDs存在一定相关性,应合理遴选和使用抗菌药物,减缓耐药菌株的产生。
Objective To explore the correlation between the resistance rate of Enterobacteriaceae bacteria and the use of antimicrobial drugs, so as to provide a guidance for the clinical rational medication of antimicrobial drugs.Methods The bacterial resistance rates of Escherichia coli and Klebsiella pneumoniae strains and antimicrobial agent consumption from 2016 to 2020 were summariaed by the excel software. Pearson correlation analysis was performed between antimicrobial drug consumption(DDDs) and bacterial resistance rates using PASW statistics18.Results The resistance rates of Klebsiella pneumoniae to piperacillin sodium and tazobactam sodium, ceftriaxone and meropenem were strongly and positively correlated with the use amount of these drugs at the hospital(r=0.931,P=0.021, r=0.880,P=0.049, and r=0.921, P=0.027, respectively), and the resistance rates of Escherichia coli to amikacin and meropenem were strongly correlated with the use amount of both drugs(r=0.988, P=0.002, and r=0.951,P=0.013).Conclusions There are correlation between the bacterial resistance rates of Escherichia coli and Klebsiella pneumoniae with the dosage of some antimicrobials in some degree, and antimicrobials should be selected and used rationally to reduce the production and dissemination of drug-resistant strains.
