两种子宫内膜准备方案对月经周期不规则患者冻融胚胎移植妊娠结局的影响 被引量:1

Effects of two endometrial preparation protocols on pregnancy outcomes in frozen-thawed embryo transfer cycles in patients with irregular menstrual cycle

摘要 目的探讨激素替代周期方案和促排卵方案准备内膜,对月经不规则患者冻融胚胎移植(FET)助孕结局的影响。方法采用前瞻性随机对照研究方法,选取2010年1月至2017年12月于中山大学孙逸仙纪念医院生殖中心行FET的月经不规则、年龄≤40岁、可利用冷冻胚胎≥3的192例患者。将患者随机分为激素替代周期组(HRT组,n=105)和促排卵方案组(OI组,n=87)。比较两组患者的一般资料、临床结局以及新生儿出生情况。结果HRT组和OI组的活产率分别为36.19%和44.83%,组间比较无统计学差异(P>0.05);两组间临床妊娠率、胚胎种植率、早期流产率、晚期流产率、多胎妊娠率、早产率、低出生体重儿率比较均无统计学差异差异(P>0.05)。OI组的内膜厚度[10.00(9.00,11.00)mm]显著大于HRT组[8.40(8.00,9.10)mm](P<0.05)。结论对于月经不规则患者FET周期,HRT方案与OI方案准备内膜的妊娠结局相当。 Objective:To explore the effects of hormone replacement therapy(HRT)protocol and ovulation induction(OI)protocol on pregnancy outcomes of frozen-thawed embryo transfer(FET)in patients with irregular menstrual cycle.Methods:A prospective randomized controlled study was performed,and 192 patients who underwent FET cycles with irregular menstruation,age≤40 years old,and available frozen embryos≥3,were selected at the Reproductive Medicine Center of Sun Yat-sen Memorial Hospital in Sun Yat-sen University from January 2010 to December 2017.The patients were randomly divided into hormone replacement cycle group(HRT group,n=105)and ovulation induction group(OI group,n=87).The general characteristics,pregnancy outcomes,neonatal outcomes were compared between the two groups.Results:The live birth rate of HRT group and OI group was 36.19%and 44.83%respectively,and there was no significant difference between the two groups(P>0.05).There were no significant differences in clinical pregnancy rate,embryo implantation rate,early abortion rate,late abortion rate,multiple pregnancy rate,premature delivery rate and low birth weight infant rate between the two groups(P>0.05).The endometrial thickness in the OI group[10.00(9.00,11.00)mm]was significantly thicker than that in the HRT group[8.40(8.00,9.10)mm](P<0.05).Conclusions:For patients with irregular menstruation undergone FET cycle,the pregnancy outcome is similar between HRT protocol and OI protocol for preparation of endometrium.
