摘要
目的探讨血清微小核糖核酸-122-5p(miR-122-5p)、微小核糖核酸-34a(miR-34a)水平与急性颅脑损伤减压术患者临床预后的关系。方法选取2019年1月—2020年1月绵阳市中心医院接受的急性颅脑损伤减压术治疗患者83例作为研究对象,根据疾病预后情况分为预后良好组51例与预后不良组32例,同期选择体检健康人员38例作为健康组,检测3组受试者血清miR-122-5p、miR-34a水平,分析两种血清与急性颅脑损伤减压术患者临床预后的关系,再根据受试者操作特征曲线(ROC)分析疾病诊断的灵敏度、特异度。结果血清miR-122-5p、miR-34a水平健康组与预后良好组均高于预后不良组,差异具有统计学意义(P<0.005)。急性颅脑损伤与血清miR-122-5p、miR-34a水平呈正相关(P<0.001)。根据ROC曲线可知,miR-122-5p灵敏度、特异度分别为65.6%、81.6%(P<0.001),miR-34a灵敏度、特异度分别为84.4%、63.2%(P<0.000),miR-122-5联合miR-34a灵敏度、特异度分别为87.5%、86.8%(P<0.000)。结论血清miR-122-5、miR-34a与急性颅脑损伤减压术患者临床预后有关,可以作为评估急性颅脑损伤减压术患者临床预后的主要方法,且miR-122-5联合miR-34a检测效果更好。
Objective To investigate the relationship between serum microRNA-122-5p(miR-122-5p)and microRNA-34a(miR-34a)levels and clinical prognosis of patients with acute head injury who underwent decompression.Methods Eighty-three patients with acute craniocerebral injury were hospitalized and underwent decompression.Next morning peripheral fasting venous blood samples were collected to measure the serum levels of miR-122-5p and miR-34a.The patients were followed up by telephone for 3 weeks to know the outcomes and then divided into 2 groups:good prognosis group(n=51)and poor prognosis group(n=32).Thirty-eight healthy subjects who underwent physical examination at the same period were selected as the healthy controls.One-way ANOVA was used to analyze the relationship between the two sera and the clinical prognosis of patients.receiver operating characteristic curve(ROC)was used to analyze the assess the prognosis.Results The serum levels of miR-122-5p and miR-34a in the healthy group were significantly higher than those in the poor prognosis group(P<0.005).Acute brain injury was positively correlated with the levels of serum mir-122-5p and miR-34a(both P<0.001).According to the ROC curve,the sensitivity and specificity of miR-122-5p were 65.6%and 81.6%(P<0.001),the sensitivity and specificity of miR-34a were 84.4%and 63.2%(P<0.000),and the sensitivity and specificity of miR-122-5 combined with miR-34a were 87.5%and 86.8%(P<0.000),respectively.Conclusion Serum miR-122-5 and miR-34a are related to the clinical prognosis of patients with acute craniocerebral injury undergoing decompression.They can be used as the main tools to evaluate the clinical prognosis of patients with acute craniocerebral injury undergoing decompression,and miR-122-5 combined with miR-34a has a better detection effect.
