摘要
目的:研究中重度慢性牙周炎患者采用激光辅助龈下刮治和根面平整术治疗对牙周组织和牙龈微循环的影响。方法:选取2018年12月-2020年12月常熟市第一人民医院收治的100例中重度慢性牙周炎患者,按随机数字表法分为两组。试验组(n=51)采取根面平整术(scaling and root planning,SRP)联合激光治疗,对照组(n=49)单纯应用SRP治疗。治疗前,治疗后2、4周患者均用激光多普勒血流仪(laser Doppler flowmetry,LDF)检查牙龈微循环,并对患者行牙周袋探诊深度(probing depth,PD)和探诊出血(bleeding on probing,BOP)检查。结果:治疗后2、4周,两组PD水平及BOP阳性率均较治疗前显著下降,且试验组均显著低于对照组(P<0.05);治疗后2、4周,与对照组比较,试验组织微血管血流灌注、活动血细胞浓度、微血管血流相对速度均显著升高,临床附着水平显著下降,差异均有统计学意义(P<0.05)。结论:Nd:YAG激光联合SRP治疗中重度慢性牙周炎的效果优于单独SRP治疗,其能更有效地清除菌斑及病变组织,改善患者牙周健康状况。
Objective:To investigate the effects of laser-assisted subgingival curettage and scaling and root planning on periodontium and gingival microcirculation in patients with moderate and severe chronic periodontitis.Method:A total of 100 patients with moderate and severe chronic periodontitis who were admitted to Changshu NO.1 People's Hospital from December 2018 to December 2020 were selected and divided into two groups according to the random number table method.The experimental group(n=51)was treated with scaling and root planning(SRP)combined with laser,and the control group(n=49)was only treated with SRP.Before treatment and 2,4 weeks after treatment,the gingival microcirculation was examined by laser Doppler flowmetry(LDF),and the probing depth(PD)and bleeding on probing(BOP)of periodontal pocket were examined.Result:After 2 and 4 weeks of treatment,the levels of PD and the positive rates of BOP in the two groups were significantly lower than those before treatment,and those in the experimental group were significantly lower than those in the control group(P<0.05).After 2 and 4 weeks of treatment,compared with the control group,the microvascular blood perfusion,the concentration of active blood cells and the relative velocity of microvascular blood flow in the experimental group significantly increased,and the clinical attachment level significantly decreased,the differences were statistically significant(P<0.05).Conclusion:The effect of Nd:YAG laser combined with SRP in the treatment of moderate and severe chronic periodontitis is better than that of SRP alone,which can remove plaque and diseased tissues more effectively and improve the periodontal health of patients.
