摘要
目的:观察并评价高血压脑卒中后失眠患者应用耳针联合针刺治疗对改善其血压与失眠症状所发挥的作用。方法:回顾性分析2020年1月-2022年3月中国人民解放军陆军第七十三集团军医院收治的82例高血压脑卒中患者临床资料,根据治疗方式的差异将患者分成对照组(n=41)与观察组(n=41),对照组采用西药联合醒神开窍针刺疗法,观察组在接受西药治疗的基础上联合应用耳针加刺人迎穴,治疗4周,比较两组血压、血脂、睡眠质量以及神经功能缺损改善效果、治疗后不良反应。结果:治疗前两组血压、血脂水平差异均无统计学意义(P>0.05),治疗后观察组收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、甘油三酯(TG)与低密度脂蛋白胆固醇(LDL-C)水平均低于对照组,高密度脂蛋白胆固醇(HDL-C)水平高于对照组,两组血压与血脂水平差异均有统计学意义(P<0.05)。治疗前匹兹堡睡眠质量指数量表(PSQI)评分与美国国立卫生院卒中量表(NIHSS)评分组间对比差异均无统计学意义(P>0.05),治疗后观察组PSQI评分及NIHSS评分均较对照组低(P<0.05)。治疗后患者不良反应发生率组间对比差异无统计学意义(P>0.05)。结论:高血压脑卒中后失眠患者应用耳针联合针刺治疗可有效改善其血压水平及睡眠质量,值得临床应用。
Objective:To observe and evaluate the effect of auricular acupuncture combined with acupuncture on improving blood pressure and insomnia symptoms in patients with insomnia after hypertensive stroke.Method:The clinical data of 82 patients with hypertensive stroke admitted to the 73rd Army Hospital of the People's Liberation Army from January 2020 to March 2022 were reviewed.According to the difference of treatment methods,the patients were divided into the control group(n=41)and the observation group(n=41).The control group was treated with western medicine combined with awakening and resuscitation acupuncture.The observation group was treated with auricular acupuncture and pricking Renying point on the basis of western medicine for 4 weeks.The levels of blood pressure and blood lipid,quality of sleep,the improvement effect of nerve function defect and the adverse reaction after treatment in two groups were compared.Result:There were no significant differences in blood pressure and blood lipid levels between the two groups before treatment(P>0.05).After treatment,the levels of SBP,DBP,TC,TG and LDL-C in the observation group were lower than those in the control group,and the level of HDL-C was higher than that in the control group,there were significant differences in blood pressure and blood lipid levels between the two groups(P<0.05).There were no significant differences between the PSQI score and NIHSS score between the two groups before treatment(P>0.05),and the PSQI scores and NIHSS scores of the observation group were lower than those of the control group after treatment(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups after treatment(P>0.05).Conclusion:Auricular acupuncture combined with acupuncture can effectively improve the blood pressure level and sleep quality of patients with insomnia after hypertensive stroke,which is worthy of clinical application.
