摘要
目的比较不同置换术治疗高龄股骨颈骨折的疗效以及存活质量的影响。方法收集2020年2月至2021年2月于我院行手术治疗股骨颈骨折高龄患者临床资料,依据髋关节置术式不同将其分为半髋关节置换术组与全髋关节置换术组,比较2组患者术后疗效与存活质量差异。结果半髋关节置换术组术后近期疗效的率为96%(44/46)高于全髋关节置换术组的93%(29/30)(χ^(2)=4.60,P=0.032);半髋关节置换术组患者术中出血量、手术时间、平均住院时间术后开始活动时间均明显低于全髋关节置换术组(P<0.05);与全髋关节置换术组相比,半髋关节置换术组患者术后1周、术后3个月的Harris评分均显著降低(P<0.05),而术后6个月与术后12个月的Harris评分比较,差异无统计学意义(P>0.05);半髋关节置换术组患者术后3个月的生存质量明显高于全髋关节置换术组(P<0.05),而其术后6个月的生存质量与全髋关节置换术组比较差异无统计学意义(P>0.05)。结论半髋关节置换术对高龄股骨颈骨骨折的治疗近期疗效优于全髋关节置换术。
Objective To compare the curative effect and the influences on survival quality of different replacement surgery on femoral neck fractures in elderly people.Methods The clinical data of elderly patients with femoral neck fractures who underwent surgery in the hospital from February 2020 to February 2021 were collected.According to different hip replacement methods,they were divided into the hemiarthroplasty group and the total hip arthroplasty group.The differences in postoperative curative effect and survival quality were compared between the two groups.Results The postoperative short-term curative effect rate in the hemiarthroplasty group was higher than that in the total hip arthroplasty group[96%(44/46)vs 93%(29/30)](χ^(2)=4.60,P=0.032).The intraoperative blood loss,operation time,average hospitalization time and postoperative activity time in the hemiarthroplasty group were significantly lower than those in the total hip arthroplasty group(P<0.05).Compared with the total hip arthroplasty group,Harris scores at 1 week and 3 months after surgery were significantly decreased in the hemiarthroplasty group(P<0.05).There was no significant difference in Harris score at 6 months and 12 months after surgery between the two groups(P>0.05).At 3 months after surgery,survival quality in the hemiarthroplasty group was significantly higher than that in the total hip arthroplasty group(P<0.05).There was no significant difference in survival quality at 6 months after surgery between the two groups(P>0.05).Conclusion The short-term curative effect of hemiarthroplasty is superior to that of total hip arthroplasty on femoral neck fractures.
