摘要
目的 分析苦芩煎剂联合甘草油治疗特应性皮炎(AD)疗效及对皮肤组织蛋白酶激活受体2(PAR-2)、瞬时型感受器电位A1离子通道(TRPA1)表达的影响。方法 选择2020年6月至2021年6于我院收治的AD患者100例为观察对象,采用数字表法分为对照组及观察组,对照组采用局部外用糖皮质激素联合抗组胺药治疗,观察组采用苦芩煎剂联合甘草油治疗。比较2组患者疗效,采集2组患者皮损组织,采用实时定量聚合酶链反应(RT-PCR)法及蛋白印迹法检测患者皮损组织中PAR-2、TRPA1 m RNA及蛋白表达变化,比较2组患者治疗前后中医症状积分,比较2组患者AD积分指数(SCORAD)评分、瘙痒程度视觉模拟评分(VAS)评分水平变化。结果 观察组患者治疗总有效率为80%,明显高于对照组的60%(P<0.05)。治疗后观察组患者皮肤干燥程度、瘙痒程度、皮损程度中医症状积分明显低于对照组(P<0.05)。治疗后2组患者SCORAD评分、VAS评分水平明显低于同组治疗前,治疗后观察组患者SCORAD评分、VAS评分水平明显低于对照组(P<0.05)。治疗后2组患者PAR-2、TRPA1 mRNA及蛋白水平明显低于同组治疗前,治疗后观察组患者PAR-2、TRPA1 m RNA及蛋白水平明显低于对照组(P<0.05)。结论 苦芩煎剂联合甘草油治疗AD患者效果显著,可通过介导皮损组织PAR-2、TRPA1的表达调控非组胺依赖性神经信号传导过程,进而缓解患者病情,可在临床中广泛应用。
Objective To analyze the efficacy of Kuqin decoction combined with licorice oil in the treatment of atopic dermatitis(AD)and its effect on the expression of the cathepsin-activated receptor(PAR)-2 and transient receptor potential A1 ion channel(TRPA1).Methods One hundred patients with AD admitted to our hospital from June 2020 to June 2021 were selected as observation subjects,and were divided into control group and observation group using the number table method.The control group was treated with topical glucocorticoids combined with antihistamines,and the observation group was treated with Kuqin decoction combined with licorice oil.To compare the efficacy of the two groups,the skin lesion tissues of the two groups were collected to detect the changes of PAR-2 and TRPA1 mRNA and protein expression by RT-PCR and immunoblotting,and the traditional Chinese medicine(TCM)symptom scores before and after the treatment of the two groups were compared,and the changes of AD score index(SCORAD)score and pruritus degree(VAS)score levels of the two groups were compared.Results The total effective rate of treatment for patients in the observation group was 80%,which was significantly higher than 60%in the control group(P<0.05).The TCM symptom scores of the degree of dry skin,itchiness and lesions in both groups after treatment were significantly lower than those before treatment in the same group,and the TCM symptom scores of the degree of dry skin,itchiness and lesions in the observation group after treatment were significantly lower than those in the control group(P<0.05).The levels of SCORAD score and VAS score of patients in both groups after treatment were significantly lower than those before treatment in the same group,and the levels of SCORAD score and VAS score of patients in the observation group after treatment were significantly lower than those in the control group(P<0.05).The levels of PAR-2,TRPA1 mRNA and protein in both groups were significantly lower than those before treatment in the same group,and the levels of PAR-2,TRPA1 mRNA and protein in the observation group were significantly lower than those in the control group after treatment(P<0.05).Conclusion Kuqin decoction combined with licorice oil is effective in treating AD patients,and can regulate the non-histamine-dependent neural signaling process through mediating the expression of PAR-2 and TRPA1 in skin lesion tissue,thus alleviating the condition of the AD patients,and can be widely used in clinical practice.
