摘要
肥胖合并抑郁现在已成为治未病领域常见的疾病,发病率逐年递增且对人们的生活和工作产生影响。王莉教授认为肥胖合并抑郁的病机多为肝气不舒,久郁伤脾,脾虚湿聚,汇聚成痰,从肝脾同治,从气论治和调气血化痰湿这几个角度入手,结合古代和现代众多医家的观点,基于整体观念和辨证论治,自拟柴胡疏肝散合四君子汤加减用以治疗疾病。柴胡舒肝散疏肝解郁,理气止痛,四君子汤健脾利湿,补脾益气,两者相结合在治愈患者上疗效显著。根据其临床的指导意义,随证加减,用药灵活。本文从该病的病因病机,论治角度,方药治疗和病案分析这几个方面进行阐述,以期为临床进一步研究奠定基础。
Obesity complicated with depression has now become a common disease in the field of treatment of pre-diseases,and its incidence is increasing year by year and has affected people’s life and work.Pro.WANG Li believes that the pathogenesis of obesity complicated with depression is mostly due to the stagnation of liver Qi causing prolonged depression and spleen deficiency,followed by dampness converging into phlegm.Starting from the perspectives of treating the liver and spleen together,treatment based on Qi,regulating Qi-flowing for eliminating phlegm,and combining the views of many ancient and modern medical practitioners,she formulated a prescription of Chaihu Shugan powder(powder of bupleuri for dispersing the depressed liver-energy)plus modified Sijunzi decoction for the treatment of this disease based on the holistic concept and dialectical treatment.The combination of Chaihu Shugan powder and Sijunzi decoction is significantly effective in curing patients,for the effects of Chaihu Shugan powder in dispersing stagnated liver Qi and regulating Qi to alleviate pain,and Sijunzi decoction in draining dampness by invigorating spleen and tonifying spleen for nourishing Qi.According to its clinical guiding significance,the dose of medication is flexible based on the syndrome.This paper elaborates on the etiology of obesity complicated with depression,the perspective of dialectical treatment,prescription treatment and case analysis are elaborated in order to lay a foundation for further clinical studies.
