摘要
目的探讨益气养心安神汤联合帕罗西汀对抑郁症失眠气郁痰阻证患者汉密尔顿抑郁量表(HAMD)、临床总体印象量表(CGI)评分的影响。方法110例抑郁症失眠气郁痰阻证患者随机分为研究组和对照组,每组55例,分别给予益气养心安神汤联合帕罗西汀和单一帕罗西汀治疗,均治疗2个月。治疗结束比较其疗效及治疗前后中医证候积分,采用匹茨堡睡眠质量指数量表(PSQI)、失眠严重程度指数(ISI)评价其睡眠质量,同时应用HAMD、CGI量表分析其情绪、症状改善情况,并记录不良反应。结果研究组临床总有效率(92.73%)显著高于对照组(76.36%)(P<0.05);两组治疗后中医证候积分(情绪低落、急躁易怒、失眠多梦、胸闷气短、神疲)比同组治疗前均明显降低,且研究组低于对照组(P<0.05);研究组治疗后PSQI评分(7.93±1.89)分、ISI评分(9.42±1.51)分、HAMD评分(10.57±1.90)分、CGI评分(2.43±0.28)分均显著低于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论益气养心安神汤联合帕罗西汀治疗抑郁症失眠气郁痰阻证,能有效改善患者抑郁症状,提高其睡眠质量,降低HAMD、CGI评分。
Objective To investigate the effect of YiqiYangxinAnshen Decoction(益气养心安神汤)combined with paroxetine on the Hamilton Depression Scale(HAMD)and Clinical Global Impression Scale(CGI)scores of patients with depression and in⁃somnia of Qi depression and phlegm obstruction syndrome.Methods A total of 110 patients with Qi depression and phlegm ob⁃struction syndrome were randomly divided into study group and control group,55 cases in each group.The two groups were respec⁃tively treated with Yiqi Yangxin Anshen Decoction combined with paroxetine and paroxetine alone for 2 months.The curative effect and TCM syndrome scores before and after treatment were compared between the two groups.The Pittsburgh Sleep Quality Index(PSQI)and Insomnia Severity Index(ISI)were used to evaluate sleep quality.Meanwhile,the scores of HAMD and CGI were used to analyze the improvement of emotions and symptoms.Adverse reactions were recorded.Results The total clinical re⁃sponse rate in the study group(92.73%)was significantly higher than that in the control group(76.36%)(P<0.05).After treatment,the scores of TCM syndromes(emotional depression,irascibility,insomnia,chest tightness,shortness of breath,fatigue)were significantly reduced in the two groups,and the above scores of the study group were lower than those of the control group(P<0.05).After treatment,the PSQI score,ISI score,HAMD score and CGI score of the study group were(7.93±1.89)points,(9.42±1.51)points,(10.57±1.90)points and(2.43±0.28)points,significantly lower than those of the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Yiqi Yangxin Anshen Decoction combined with paroxetine can effectively improve the depressive symptoms,improve sleep quality and reduce HAMD and CGI scores of patients with depression and insomnia of Qi depression and phlegm obstruction syndrome.
