摘要
目的 探讨冠心病患者心率变异性(HRV)与认知功能的相关性。方法 选取北京航天总医院心内科2021年12月至2022年12月收治的385例冠心病患者作为研究对象,对所有患者动态心电图及认知功能指标进行分析,根据蒙特利尔认知评估量表(MOCA)将认知功能分为认知功能障碍组(MOCA评分<23分)95例、轻度认知功能障碍组(MOCA评分23~25分)161例、无认知功能障碍组(MOCA评分≥26分)129例。分析HRV与认知功能的关系。结果 相邻NN间期的总体标准差(SDNN)与认知功能呈线性负相关,Spearman相关性检验P=0.036,差异有显著统计学意义(P<0.05),SDNN与认知功能线性回归分析得出的回归系数为-0.304,SDNN和MOCA评分应呈负向相关;相邻NN间期的均值(NNS)、相邻NN间期差值的均方根(RMSSD)、NN间期与平均NN间期差值大于50 ms的心搏数占总心搏数的比例(PNN50)、低频(LF)、高频(HF)、低频/高频比(LF/HF)与MOCA评分均无相关性。结论 SDNN越大,MOCA评分越低,认知障碍越严重。
Objective To explore the correlation between heart rate variability(HRV)and cognitive function in patients with coronary heart disease.Methods A total of 385 patients with coronary heart disease admitted by the Department of Cardiology of Beijing Aerospace General Hospital from December 2021 to December 2022 were selected as the research objects,and the Holter and cognitive function indicators of all patients were analyzed.According to the Montreal Cognitive Assessment Scale(MOCA),the patients were divided into the cognitive dysfunction group(MOCA Score<23)with 95 cases,mild cognitive dysfunction group(MOCA Score 23~25)with 161 cases,and no cognitive dysfunction group(MOCA Score≥26)with 129 cases.The relationship between HRV and cognitive function was analyzed.Results The standard diviation of NN intervals(SDNN)was linearly negatively correlated with cognitive function with Spearman correlation test P=0.036,the difference was statistically significant(P<0.05);the regression coefficient obtained by linear regression analysis of SDNN and cognitive function was-0.304,and SDNN and MOCA scores should be negatively correlated.There was no correlation between MOCA scores and the mean value of adjacent NN intervals(NNS),root mean square successive difference(RMSSD),proportion of the number of heart beats with the difference between NN interval and average NN interval greater than 50 ms to the total number of heart beats(PNN50),low frequency(LF),high frequency(HF)and low frequency/high frequency ratio(LF/HF).Conclusion The larger the SDNN,the lower the MOCA score and the more serious the cognitive impairment.
