摘要
目的探讨负压封闭引流(VSD)联合重组牛碱性成纤维细胞生长因子(rb-bFGF)在慢性难愈合创面中的应用效果。方法选取2020年2月至2022年2月许昌中医院收治的100例慢性难愈合创面患者作为研究对象,按照不同治疗方法将其分为联合组(50例)和VSD组(50例),联合组患者局部创面采用VSD联合rb-bFGF治疗,VSD组患者局部创面单纯采用VSD治疗,对比观察两组患者创面肉芽组织成熟度、氧分压、血清乳酸脱氢酶(LDH)与琥珀酸脱氢酶(SDH)水平及VSD敷料更换次数。结果治疗2周后,联合组患者肉芽组织成熟度为Ⅰ度3例、Ⅱ度11例、Ⅲ度15例、Ⅳ度21例,明显优于VSD组患者的肉芽组织成熟度为Ⅰ度17例、Ⅱ度8例、Ⅲ度16例、Ⅳ度9例(Z=-3.270,P<0.001);治疗2周后,联合组患者创面氧分压及血清SDH水平均明显高于VSD组(t=17.682、2.653,P<0.001、P=0.009),而LDH水平明显低于VSD组(t=10.038,P<0.001);联合组患者VSD敷料更换次数为(3.62±0.53)次,明显少于VSD组患者的VSD敷料更换次数(5.13±0.61)次(t=13.210,P<0.001)。结论与单纯应用VSD治疗相比,VSD联合rb-bFGF治疗更有利于提高慢性难愈合创面肉芽组织成熟度及SDH活性,降低LDH活性,改善创面局部微环境,缩短治疗时间。
Objective To discuss the clinical efficacy of vacuum sealing drainage(VSD)combined with recombinant bovine basic fibroblast growth factor(rb⁃bFGF)in the treatment of chronic refractory wounds.Methods 100 patients with chronic refractory wounds,admitted to Xuchang Hospital of Traditional Chinese Medicine from February 2020 to February 2022,were enrolled as subjects,and divided into combination group(n=50),receiving the combined treatment of VSD and rb⁃bFGF,and VSD group(n=50),receiving the treatment of VSD alone.The maturity of granulation tissues,oxygen partial pressure,levels of serum lactate dehydrogenase(LDH)and serum succinate dehydrogenase(SDH),and the number of changing VSD dressing were compared between the two groups.Result After 2 weeks of treatment,the maturity of granulation tissues of patients in the combination group was evaluated as gradeⅠin 3 cases,gradeⅡin 11 cases,gradeⅢin 15 cases,and gradeⅣin 21 cases,being obviously superior to that in the VSD group⁃17 cases gradeⅠ,8 cases gradeⅡ,16 cases gradeⅢ,and 9 cases gradeⅣ(Z=-3.270,P<0.001);the oxygen partial pressure and the level of serum SDH of patients in the combination group were obviously higher compared with the VSD group(t=17.682 and 2.653,P<0.001,P=0.009),while the level of LDH in the combination group was obviously lower than that of the VSD group(t=10.038,P<0.001);the number of changing VSD dressing in the combination group was(3.62±0.53)times,which was obviously fewer than(5.13±0.61)times in the VSD group(t=13.210,P<0.001).Conclusion Compared with the alone application of VSD,the combined application of VSD and rb⁃bFGF in the treatment of chronic refractory wounds is more favorable for improving the maturity of granulation tissue and SDH activity,reducing LDH activity,ameliora⁃ting the local microenvironment of the wound,shortening the treatment time therefore.
