摘要
目的观察处方前置审核系统规则维护联合处方点评对急诊儿科处方质量的影响,促进临床合理用药。方法江苏省宿迁市第一人民医院药学部通过事后处方点评、急诊药房窗口的拦截结果、系统审核,评价急诊儿科不合理处方的拦截效果。结果2020年3—7月(处方前置审核系统应用前)急诊儿科处方合理率为90.20%,2021年3—7月(处方前置审核系统应用后)处方合理率为97.30%,处方合理率提高7.10%,不合理处方率明显下降;急诊药房窗口拦截不合理处方率由5.59%降至2.24%;2021年3—7月临床医师主动修改的处方67张,占系统审核总处方数的0.13%,医师主动修改率为65.30%;经由医师自愿提交进入系统经药师人工审核的处方1312张,占系统审核总处方数的2.52%;药师干预问题处方成功率为80.60%。结论急诊儿科的前置审方的顺利开展可提高处方前置审核系统审方的准确率,提升合理用药水平,提高工作效率。
Objective To observe the influence of prescription pre-audit system rules maintenance combined with prescription comments on the quality of emergency pediatric prescriptions,and promote clinical rational drug use.Methods The pharmacy department of Suqian First People′s Hospital,Jiangsu Province,evaluated the interception effect of unreasonable prescriptions in emergency department of pediatrics through post prescription comments,interception results of emergency pharmacy windows,and system audits.Results From March to July 2020(before the application of the prescription pre-audit system),the rational rate of emergency pediatric prescriptions was 90.20%,and from March to July 2021(after the application of the prescription pre-audit system),the rational rate of prescriptions was 97.30%,the rational rate of prescriptions increased by 7.10%,and the rate of irrational prescriptions decreased significantly;The rate of unreasonable prescriptions intercepted at the emergency pharmacy window decreased from 5.59%to 2.24%;From March to July 2021,67 prescriptions were actively modified by clinicians,accounting for 0.13%of the total prescriptions reviewed by the system,and the rate of active modification by physicians was 65.30%;1312 prescriptions entered the system voluntarily submitted by physicians and manually reviewed by pharmacists,accounting for 2.52%of the total prescriptions reviewed by the system;The success rate of pharmacists′intervention in problem prescriptions was 80.60%.Conclusion The smooth implementation of the pre-prescription review of emergency pediatrics can improve the accuracy of the prescription pre review system,improve the level of rational drug use,and improve work efficiency.
