摘要
目的:探讨运用电热针联合低频重复经颅磁刺激(LF-rTMS)在脑卒中伴屈肘肌痉挛患者中的治疗效果。方法:选取符合条件的脑卒中患者60例作为研究对象,随机分为对照组和观察组各30例。对照组在常规康复训练基础上实施LF-rTMS治疗,观察组在对照组基础上增加电热针治疗,比较治疗前及治疗4周后2组患者偏侧屈肘肌肌张力改良Ashworth评级(MAS)、上肢Fugl-Meyer评分(UFMA)、日常生活活动能力(ADL)、运动诱发电位(MEP)潜伏期及波幅。结果:治疗后,2组患者MAS评级均有改善,且观察组优于对照组(均P<0.05),UFMA评分及ADL评分均较治疗前增加,且观察组均高于对照组(均P<0.05);MEP潜伏期治疗前后及组间比较差异均无统计学意义;对照组治疗前后波幅变化不明显,但观察组治疗后波幅增加,且明显治疗后高于对照组(均P<0.05)。结论:电热针联合低频rTMS对脑卒中屈肘肌痉挛疗效更佳,可明显促进患者上肢运动功能康复、提高患者日常生活活动能力。
Objective:To investigate the effect of electrothermal needle combined with low-frequency repetitive transcranial magnetic stimulation(LF-rTMS)in stroke patients with elbow flexor spasm.Methods:A total of 60 eligible stroke patients treated in the Rehabilitation Department of Wuhan Puren Hospital from August 2021 to August 2022 were selected as the research objects.They were randomly divided into control group and combined group with 30 cases in each group.The control group was treated with low-frequency rTMS on the basis of routine rehabilitation training,and the combined group was treated with electrothermal acupuncture on the basis of the control group.The muscle tension modified-ashworth scale(MAS),upper limb Fugl-meyer assessment(UFMA),activity of daily living(ADL),motor evoked potential(MEP)latency and amplitude of the two groups were compared before and 4 weeks after treatment.Results:After treatment,the MAS score,UFMA score and ADL score of the two groups were significantly improved,but the improvement in the combined group was more significant than that in the control group(P<0.05).There was no significant difference in MEP latency before and after treatment and between two grou.The amplitude of the control group did not change significantly before and after treatment,but the amplitude of the combined group increased after treatment,and was significantly higher than that of the control group after treatment(P<0.05).Conclusion:Electrothermal acupuncture combined with low-frequency r-TMS has a better effect on stroke patients with elbow flexor spasm,which can significantly promote the rehabilitation of upper limb motor function and improve the ADL activities of patients.
