SF、HCY和LP-PLA2与急性脑梗死相关性研究 被引量:1

Correlation Analysis of Serum Ferritin,Homocysteine,Lipoprotein Associated Phospholipase A2 and Acute Cerebral Infarction

摘要 目的 检测急性脑梗死患者血清铁蛋白(SF)、同型半胱氨酸(HCY)和脂蛋白相关凝脂酶A2(LP-PLA2)水平,探讨其与急性脑梗死(ACI)相关性及其临床价值。方法 选取我院2018年1月至2019年10月171例ACI患者作为研究组,根据病情分为大梗死组、中梗死组、小梗死组及腔隙性脑梗死组;同时选取同期的健康体检者50例作为对照组。采用磁微粒化学发光法检测SF、循环酶法检测HCY及电化学发光免疫法检测LP-PLA2水平,并对其进行比较,分析其与脑梗死相关性,对比单项及SF、HCY、LP-PLA2三项联合检测诊断ACI灵敏度及特异度。结果 研究组SF、HCY和LP-PLA2水平均高于对照组(P均<0.05);大梗死组SF、HCY和LP-PLA2水平均高于中梗死、小梗死及腔隙性脑梗死组(P均<0.05),中梗死与小梗死组SF、HCY和LP-PLA2水平均无明显差异(P均>0.05),中梗死、小梗死组SF、HCY和LP-PLA2水平均高于腔隙性脑梗死组(P均<0.05);SF、HCY及LP-PLA2三者联合检测ACI灵敏度是64.7%(110/171),特异度是98%(49/50),三者联合检测灵敏度均高于三者单项检测(P均<0.05)。结论 SF、HCY和LP-PLA2与ACI有明显相关性,且与脑梗死面积密切相关,三者联合检测可提高诊断ACI梗死面积的灵敏度及特异度,对ACI严重程度及临床预后的预判有重要临床价值。 Objective To detect serum ferritin(SF),homocysteine(HCY),lipoprotein associated phospholipase A2(LP-PLA2)in acute cerebral infarction(ACI)patients,and to investigate their relationship with ACI and clinical values.Methods From January 2018 to October 2019,171 ACI patients in our hospital were selected as study group,and patients were divided into large infarction group,medium infarction group,small infarction group and lacunar infarction group.Meanwhile,50 healthy people in the same period were selected as control group.SF was detected by magnetic particle chemiluminescence method,HCY was detected by circulating enzyme method and LP-PLA2 was detected by electrochemical luminescence immunoassay.The correlation between SF、HCY、LP-PLA2 and cerebral infarction was analyzed,and sensitivity and specificity of SF,HCY and LP-PLA2 in diagnosis of ACI were analyzed.Results SF,HCY and LP-PLA2 in study group were higher than control group(P<0.05).SF,HCY and LP-PLA2 in the large infarction group were higher than medium infarction group,small infarction group and lacunar infarction group(P<0.05).There was no significant difference in SF,HCY and LP-PLA2 between medium infarction group and small infarction groups(P>0.05).SF,HCY and LP-PLA2 in medium and small infarction groups were all higher than lacunar infarction group(P<0.05).Sensitivity and specificity of combined detection of SF,HCY and LP-PLA2 were 64.7%(110/171)and 98%(49/50),respectively.Sensitivity of combined detection of SF,HCY and LP-PLA2 was higher than that of single detection(P<0.05).Conclusion SF,HCY and LP-PLA2 have significantly correlation with ACI that are closely related to the area of cerebral infarction.Combined detection of the three could improve sensitivity and specificity of diagnosis ACI infarct area,and would have important clinical value in predicting severity of ACI and clinical prognosis.
