摘要
目的探讨2型糖尿病(type 2 diabetes mellitus,T2DM)胰岛素抵抗患者开展耳穴埋籽辅助治疗联合黄连解毒汤加减治疗的临床效果。方法方便选取2019年6月-2021年8月常州市中医医院内分泌科收治的T2DM胰岛素抵抗患者78例为研究对象,根据数字表法分组,对照组39例患者进行常规干预联合二甲双胍治疗,观察组39例患者在对照组基础上联合开展耳穴埋籽和黄连解毒汤加减治疗。观察两组治疗前后血糖值、胰岛素值、胰岛素抵抗指标及中医证候积分。结果治疗前,两组血糖值、胰岛素值、胰岛素抵抗指标及中医证候积分比较,差异无统计学意义(P>0.05);治疗后,观察组胰岛素抵抗指数(HOMA-IR)(2.33±0.42)低于对照组,胰岛β细胞功能指数(HOMA-β)(42.14±3.26)和胰岛素敏感指数(ISI)(0.90±0.28)均高于对照组,差异有统计学意义(t=7.219、9.064、4.234,P<0.05);治疗后,观察组中医证候积分低于对照组,差异有统计学意义(P<0.05)。结论在T2DM胰岛素抵抗的治疗中应用耳穴埋籽辅助治疗联合黄连解毒汤加减治疗可改善患者的胰岛功能,有效控制血糖,值得推广应用。
Objective To investigate the clinical effects on the combination of ear-point buried seed adjuvant therapy and coptis root detoxification decoction plus and minus treatment in patients with insulin resistance in type 2 diabetes mellitus(T2DM).Methods 78 patients with T2DM insulin resistance admitted to the Endocrinology Department of Changzhou Hospital of Traditional Chinese Medicine from June 2019 to August 2021 were conveniently selected as the study subjects.Grouped according to the digital table method,39 patients in the control group were treated with conventional intervention combined with metformin,and 39 patients in the observation group were treated with ear point seed implantation and modified Huanglian Jiedu Decoction on the basis of the control group.Observe the blood glucose value,insulin value,insulin resistance index and TCM syndrome score before and after treatment in two groups.Results compared the blood glucose value,insulin value,insulin resistance index and TCM symptoms score before treatment between two groups,and the difference were not statistically significant(P>0.05).After treatment,the insulin resistance index(HOMA-IR)of the observation group(2.33±0.42)was lower than that of the control group,βcell function index(HOMA-β)(42.14±3.26)and insulin sensitivity index(ISI)(0.90±0.28)were significantly higher than those in the control group,the difference was statistically significant(t=7.219,9.064,4.234,P<0.05).After treatment,the score of TCM symptoms in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion In the treatment of insulin resistance in T2DM,the application of ear-point buried seed adjuvant therapy combined with coptis root detoxification decoction plus and minus treatment can improve the pancreatic function of patients,effectively control glucose,It is worth promoting.
