摘要
目的 比较激光、冷冻及外用药三种方法治疗面部多发性扁平疣的疗效与安全性。方法 方便选取2016年1月—2019年6月期间厦门长庚医院皮肤科诊治的105例面部多发性扁平疣患者,为研究对象,随机分成3组,各35例,按照治疗方法分为激光组(采用Er:YAG 2 940 nm激光治疗)、冷冻组(采用液态氮治疗)、外用药膏组(采用0.1%维A酸乳膏外用治疗),对比3组患者治疗效果与安全性。结果 激光组有效率明显高于冷冻组和外用药组,差异有统计学意义(χ^(2)=4.058、7.295,P<0.05);随访3个月,激光组复发率为17.14%,冷冻组复发率为42.86%,外用药组复发率为40.00%,激光治疗组复发率低于冷冻治疗组和外用药组,差异有统计学意义(χ^(2)=5.510、4.480,P<0.05);3组不良反应发生率依次为28.57%、57.14%、54.29%,激光组不良反应发生率低于其他两种方法,差异有统计学意义(P<0.05)。结论 对于面部多发性扁平疣患者患者来说,采用Er:YAG 2 940 nm激光治疗方案可以取得较好的治疗效果,复发率较低,不良反应少。
Objective To compare the efficacy and safety of laser,cryotherapy and topical medicine in the treatment of multiple facial flat warts.Methods From January 2016 to June 2019,105 patients with multiple facial flat warts diagnosed and treated in Department of Dermatology in the Xiamen Changgeng Hospital were conveniently selected as the study objects and randomly divided into three groups,with 35 patients in each group.According to the treatment method,they were divided into laser group treated with Er:YAG 2940 nm laser,freezing group treated with liquid nitrogen,and external ointment group treated with 0.1%retinoic acid cream.The treatment effect and safety of the three groups were compared.Results The effective rate of the laser group was significantly higher than that of the freezing group and the external drug group,the difference was statistically significant(χ^(2)=4.058,7.295,P<0.05).Followed up for 3 months,the recurrence rate in the laser group was 17.14%,42.86%in the freezing group,and 40.00%in the external drug group.The recurrence rate in the laser treatment group was lower than that in the cryotherapy group and the topical drug group,and the difference was statistically significant(χ^(2)=5.510,4.480,P<0.05).The incidence of adverse reactions in the three groups were 28.57%,57.14%,54.29%,respectively,and the incidence of adverse reactions in the laser group was lower than that in the other two methods,the difference was statistically significant(P<0.05).Conclusion For the patients with facial multiple flat warts,the Er:YAG 2940 nm laser treatment scheme can achieve good therapeutic effect,with low recurrence rate and less adverse reactions.
