容积旋转调强放疗在宫颈癌患者中的临床疗效及安全性 被引量:1

Clinical Efficacy and Safety of Volume Rotary Intensity Modulated Radiotherapy in Patients with Cervical Cancer

摘要 目的研究容积旋转调强放疗在宫颈癌患者中的临床疗效及安全性。方法回顾性分析2019年1月至2020年1月医院收治的80例宫颈癌患者资料,根据不同治疗方法分为观察组和对照组,观察组44例,对照组36例。两组均用锥形束CT进行引导,对照组给予适形调强放疗,观察组给予容积旋转调强放疗,两组随访时间均为2年。比较两组放疗后的远期疗效、计划靶区受照剂量分布、危及器官的受照剂量和不良反应情况。结果治疗后,观察组临床总有效率明显高于对照组,差异有统计学意义(P<0.05);观察组的平均剂量分布高于对照组,差异有统计学意义(P<0.05);两组HI和CI靶区分布的剂量值比较,差异无统计学意义(P>0.05);股骨头受照剂量比较,观察组V20数值高于对照组,差异有统计学意义(P<0.05),V_(10)、V_(30)、D_(mean)与对照组比较,差异无统计学意义(P>0.05);膀胱受照剂量比较,观察组V20剂量值均低于对照组,差异有统计学意义(P<0.05),V_(10)、V_(30)、V_(40)和V_(46)受照剂量值比较,差异无统计学意义(P>0.05);观察组直肠受照剂量V20低于对照组,差异有统计学意义(P<0.05),V_(10)、V_(30)、V_(40)及V_(46)与对照组比较,差异无统计学意义(P>0.05);观察组不良反应总发生率低于对照组,且各项急性、慢性放射性损伤的发生率均低于对照组,差异有统计学意义(P<0.05)。结论容积旋转调强放疗技术应用于宫颈癌患者能够有效提高临床治疗效果,并在靶区适形性和均匀性方面具有一定正面效果,可更好地保护邻近器官,同时减轻急、慢性放射损伤,提高安全性。 Objective The clinical efficacy and safety of volume rotary intensity modulated radiotherapy in cervical cancer patients were investigated.Methods With retrospective analysis of the data of 80 cervical cancer patients admitted to the hospital from January 2019 to January 2020,they were divided into the observation group and the control group based on different treatment methods,with 44 cases in the observation group and 36 cases in the control group.With the use of conical beam CT for guidance in both groups,the control group was treated with conformal intensity modulated radiotherapy,while the observation group was treated with volume rotary intensity modulated radiotherapy,with follow-up time for 2 years in both groups.The long-term efficacy,dose distribution in planned target area,organ threatening radiation dose,and adverse reactions of the two groups after radiotherapy were compared.Results After treatment,the total clinical effective rate of the observation group was significantly higher than that of the control group,with the statistically significant difference(P<0.05);The average dose distribution of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05);There was no statistically significant difference in the dose values of HI and CI target distribution between the two groups(P>0.05);For the comparison of irradiation doses on the femoral head,the V_(20)values in the observation group were higher than those in the control group,with the statistically significant differences(P<0.05).However,there were no statistically significant differences in V_(10),V_(10),and Dmean compared to the control group(P>0.05);For the comparison of radiation doses to the bladder,the V_(20)dose values in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05),while the differences in radiation dose values between V_(10),V_(30),V_(40)and V_(46)were not statistically significant(P>0.05);The rectal irradiation doses V_(20)in the observation group were lower than that in the control group,and the differences were statistically significant(P<0.05).There were no statistically significant differences in V_(10),V_(10),V_(40) and V_(46) between the two groups(P>0.05);The total incidence of adverse reactions in the observation group was lower than that in the control group,and the incidence of various acute and chronic radiation injuries was lower than that in the control group,with the statistically significant difference(P<0.05).Conclusions The application of volume rotary intensity modulated radiotherapy technology in cervical cancer patients can effectively improve clinical treatment effectiveness,and has a certain positive effect in target area conformability and uniformity.It can better protect adjacent organs,reduce acute and chronic radiation damage,and improve safety.
