摘要
目的探讨术前白蛋白-球蛋白比值(AGR)对结肠癌预后的临床意义。方法回顾性分析2013年1月至2017年12月在我院行根治性切除的126例结肠癌患者的临床资料。计算患者术前AGR,记录患者的无进展生存期(PFS)、总生存期(OS)。结果AGR高水平(>1.52)与低水平(≤1.52)患者TNM分期及CRP水平存在统计学差异(P<0.05)。Spearman秩相关性分析显示,结肠癌患者AGR与PNI呈正相关(r=0.483,P<0.001),与CONUT评分呈负相关(r=-0.316,P<0.001)。Kaplan-Meier分析显示AGR高水平者PFS率(82.54%)和OS率(88.89%)较低水平者(65.08%和63.49%)更高(χ^(2)=5.064,P<0.05和χ^(2)=11.165,P<0.001)。单因素及多因素Cox回归分析显示,AGR是结肠癌患者OS预后的独立影响因素(P<0.05)。结论对于计划行结肠癌根治性手术的患者,AGR可能是有效的预后标志物之一。手术前后应进行有效的营养支持,可能有助于减少手术创伤,降低术后化疗的毒副作用,延长生存期,提高患者的生活质量。
Objective To analyze the clinical value of preoperative albumin-globulin ratio(AGR)on the prognosis of colon cancer after radical resection.Methods The clinical data of 126 patients with colon cancer received radical resection in our hospital from January 2013 to December 2017 were analyzed retrospectively.The AGR was calculated,and progression-free survival(PFS)and overall survival(OS)of the patients were recorded.Results There were statistically significant differences in TNM stage and C-reactive protein level between the patients with high AGR(>1.52)and low AGR(≤1.52)(P<0.05).Spearman rank correlation analysis revealed a positive correlation between AGR and PNI in colon cancer cases(r=0.483,P<0.001),and a negative correlation between AGR and CONUT score(r=-0.316,P<0.001).Kaplan-Meier analysis showed progression-free survival rate and overall survival rate was 82.54%and 88.89%respectively in the patients with high level of AGR,both were higher than 65.08%and 63.49%respectively in the patients with low level of AGR(χ^(2)=5.064,P<0.05,andχ^(2)=11.165,P<0.001).Univariate and multivariate Cox regression analysis showed that AGR was an independent prognostic factor for OS in colon cancer patients(P<0.05).Conclusion AGR may be one of the effective prognostic markers for the colon cancer patients planning to perform radical surgery.Peri-operative nutritional support would be provided effectively to reduce operative trauma and side-effect of postoperative chemotherapy,extend the survival period and improve the quality of life for colon cancer patients.
