摘要
目的 探讨Bcl-2-like 11(BIM)基因多态性与肺间质纤维化肺功能和肺部感染的关联。方法 选择2018年5月-2019年5月河北医科大学第二医院东院收治的肺间质纤维化患者100例为研究组,另随机选择医院体检健康人60名为对照组。收集临床资料,统计研究组患者肺纤维化类型、合并感染和病原类型。分析BIM基因rs724710位点多态性,采用肺功能检测仪检测肺功能,统计患者胸部高分辨率CT(HRCT)评分。结果 BIM基因rs724710位点存在TT、TC和CC三种基因型,研究组、对照组三种基因型实际频数和理论频数分布比较,无统计学差异;研究组TC+TT基因型频率低于对照组,TC+CC基因型和C等位基因频率高于对照组(P<0.05);研究组BIM基因rs724710位点携带CC基因型患者FVC、FEV1和TLC低于携带TC+TT型患者,HRCT评分高于携带TC+TT型患者,差异具有统计学意义(P<0.05),两组合并感染、感染病原类型比较无统计学差异;研究组BIM基因rs724710位点携带TT基因型患者FVC、FEV1和TLC高于携带TC+CC型患者,HRCT评分低于携带TC+CC型患者,差异具有统计学意义(P<0.05),两组合并感染、感染病原类型比较无统计学差异。结论BIM基因rs724710位点CC基因型显著增加肺间质纤维化易感性,且对患者肺功能有负面影响,但对肺部感染易感性无明显影响。
OBJECTIVE To explore the association of Bcl-2-like 11(BIM) gene polymorphism with lung function and pulmonary infection in patients with pulmonary interstitial fibrosis. METHODS A total of 100 patients with pulmonary interstitial fibrosis who were treated in the East Hospital, the Second Hospital of Hebei Medical University from May 2018 to May 2019 were assigned as the study group, meanwhile, 60 healthy people who received physical examination were chosen as the control group. The clinical data were collected, the types of pulmonary fibrosis, complication with infection and species of pathogens were statistically analyzed for the patients of the study group. The polymorphism of BIM gene rs724710 locus was observed, the lung function was examined by lung function test device, and the scores of high resolution CT(HRCT) of chest were statistically analyzed. RESULTS There were no significant differences in the actual frequencies and theoretical frequencies of TT, TC and CC3 genotypes at BIM gene rs724710 locus between the study group and the control group;the frequency of TC+TT genotype of the study group was significantly lower than that of the control group, the frequencies of TC+CC genotype and allele C of the study group were significantly higher than those of the control group(P<0.05). In the study group, the FVC, FEV1 and TLC of the patients who carried with CC genotype at BIM gene rs724710 locus were significantly lower than those of the patients who carried with TC+TT genotype, the HRCT score of the patients who carried with CC genotype was significantly higher than that of the patients who carried with TC+TT genotype(P<0.05). There were no significant differences in the complication with infection and species of pathogens between the two groups. In the study group, the FVC, FEV1 and TLC of the patients who carried with TT genotype at BIM gene rs724710 locus were significantly higher than those of the patients who carried with TC+CC genotype, the HRCT score of the patients who carried with TT genotype was significantly lower than that of the patients who carried with TC+CC genotype(P<0.05). There were no significant difference in the complication with infection and species of pathogens between the two groups. CONCLUSION The CC genotype at BIM gene rs724710 locus may significantly increase the susceptibility to pulmonary interstitial fibrosis and has negative effect on the lung function, but it has no remarkable effect on the susceptibility to pulmonary infection.
