摘要
目的探讨磁共振T_(2)mapping序列在痛风性踝关节炎患者关节软骨损伤的应用价值。方法选取经临床确诊痛风性踝关节炎患者56例作为病例组;15例共30踝无踝关节疾病的健康志愿者作为对照组。对所有患者及健康志愿者行常规踝关节序列及T_(2)mapping序列扫描,2位放射科医师测量胫骨前区、胫骨中间区、胫骨后区、距骨前区、距骨中间区和距骨后区的软骨T_(2)值,分析比较痛风性踝关节炎患者关节软骨与正常对照组T_(2)值的差异,并对观察者组间和组内进行一致性验证。结果除了观察者1测量的胫骨前区(P=0.077)外,踝关节其余部位测量的软骨T_(2)平均值均显著高于对照组(P<0.05);观察者组间和组内的一致性均较好。结论痛风性踝关节炎患者关节软骨的T_(2)值明显高于对照组。T_(2)mapping序列可以在一定程度上反映关节软骨损伤,可作为痛风性踝关节炎早期软骨损伤的评价工具。
Objective To investigate the application value of T_(2)mapping sequence in articular cartilage injury in patients with gouty ankle arthritis.Methods 56 patients with gouty ankle arthritis clinically diagnosed were collected as the case group;15 people and 30 healthy volunteers without ankle joint disease were collected as the control group.Routine ankle joint sequence and T_(2)maping sequence scans were performed on all patients and healthy volunteers.Two radiologists measured the cartilage T_(2)values of the anterior tibia,the middle tibia,the posterior tibia,the anterior talus,the middle talus,and the posterior talus.We analyzed and compared the difference in T_(2)values of articular cartilage between patients with gouty ankle arthritis and the normal control group,and verifed the consistency between and within the observer group.Results Except for the anterior tibial area measured by observer 1(P=0.077),the average T_(2)values of cartilage measured in other parts of ankle joint were significantly higher than those in the control group(P<0.05);Both inter-and intraobserver agreements were excellent.Conclusion The T_(2)value of articular cartilage in patients with gouty arthritis is significantly higher than that in control group.T_(2)mapping sequence can reflect articular cartilage damage to a certain extent,and can be used as an evaluation tool for early cartilage damage in gouty ankle arthritis.
