摘要
目的 探讨心脏超声在慢性心力衰竭(congestive heart failure,CHF)患者中的应用价值。方法 选取2018年1月—2021年12月就诊于南京市六合区人民医院超声科的80例CHF患者(观察组)以及80名体检健康人员(对照组)为研究对象,均有完整的心脏超声检查资料,比较两组及不同NYHA心功能分级患者的超声参数差异性。结果 观察组左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室后壁厚度(LVPWd)、左房内径(LAD)、室间隔厚度(IVS)值分别为(58.13±3.56)、(46.38±3.20)、(10.12±0.51)、(38.24±3.36)、(13.32±0.38)mm,高于对照组的(46.25±2.97)、(31.21±2.15)、(8.91±0.21)、(30.12±1.21)、(9.41±0.36)mm,差异有统计学意义(t=22.919、35.195、19.622、20.337、66.811,P<0.05)。观察组左室射血分数(LVEF)、心脏舒张早期最大血流(E)/运动速度(A)高于对照组,差异有统计学意义(P<0.05)。随着NYHA心功能分级增加,LVPWd、LVEDD、LVESD、LAD、IVS逐渐增高,LVEF、E/A逐渐降低,差异有统计学意义(P<0.05)。结论 对CHF患者应用心脏超声有利于准确评价患者的心脏形态及功能变化,为临床诊断提供必要的参考依据。
Objective To explore the application value of cardiac ultrasound in patients with chronic heart failure(CHF).Methods 80 CHF patients(observation group)and 80 healthy persons(control group)who were examined in the ultrasound department of Nanjing Liuhe District People's Hospital from January 2018 to December 2021 were selected as the study subjects.All of them had complete cardiac ultrasound data,and the differences of cardiac ultrasound parameters between the two groups and patients with different NYHA cardiac function grades were compared.Results The left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular posterior wall thickness(LVPWd),left atrial diameter(LAD),and interventricular septal(IVS),in the observation group were(58.13±3.56)mm,(46.38±3.20)mm,(10.12±0.51)mm,(38.24±3.36)mm,(13.32±0.38)mm,respectively,higher than those in the control group(46.25±2.97)mm,(31.21±2.15)mm,(8.91±0.21)mm,(30.12±1.21)mm(9.41±0.36)mm,the difference was statistically significant(t=22.919,35.195,19.622,20.337,66.811,P<0.05).The left ventricular ejection fraction(LVEF)and the early diastolic maximum blood flow(E)/motion velocity(A)in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).With the increase of NYHA cardiac function grading,LVPWd,LVEDD,LVESD,LAD and IVS increased gradually,while LVEF and E/A decreased gradually,the difference was statistically significant(P<0.05).Conclusion The application of cardiac ultrasound in CHF patients is helpful to accurately evaluate the changes of cardiac morphology and function and provide necessary reference for clinical diagnosis.
