摘要
目的探究糖尿病肾损伤患者尿微量白蛋白(mA1b)和β_(2)-微球蛋白(β_(2)-MG)的诊断意义。方法选取2020年3月—2022年3月厦门大学附属中山医院收治的120例糖尿病患者作为研究对象,按照是否出现肾损害分成两组,每组60例。对照组为未出现肾损伤者,研究组为存在肾损伤者。对比两组的各项肾功能指标,阳性率以及各年龄阶段的mAlb、β_(2)-MG指标水平,分析年龄与mAlb、β_(2)-MG的相关性。结果研究组的尿mAlb、β_(2)-MG指标水平均高于对照组,差异有统计学意义(P<0.05)。mAlb、β_(2)-MG联合检测的临床阳性率高于单一的mAlb、β_(2)-MG诊断率,差异有统计学意义(P<0.05);mAlb的临床阳性率高于β_(2)-MG,差异有统计学意义(P<0.05)。各年龄阶段的mAlb、β_(2)-MG水平逐渐增加,差异有统计学意义(P<0.05)。30~40岁、41~50岁、51~60岁年龄段与mAlb、β_(2)-MG水平具有正相关性(P<0.05)。结论mAlb和β_(2)-MG指标在糖尿病检测工作中极为重要,两种指标联合检测可以大幅度提升肾损伤情况的检出率,表明随着年龄的增长各项指标水平逐渐提高,值得推广应用。
Objective To investigate the diagnostic significance of urinary microalbumin(mAlb)andβ_(2)-MG in patients with diabetic kidney injury.Methods 120 diabetic patients admitted to Zhongshan Hospital Affiliated to Xiamen University from March 2020 to March 2022 were selected as the research object and divided into two groups,with 60 cases in each group according to whether kidney damage occurred.The control group had no kidney injury,and the study group had kidney injury.The renal function indexes,positive rate and mAlb andβ_(2)-MG indexes of patients at different ages were compared,analyze age and mAlb andβ_(2)-MG relevance.Results Urinary mAlb andβ_(2)-MG index level were significantly higher than those of the control group,the difference was statistically significant(P<0.05).The clinical positive diagnosis rate of mAlb,β_(2)-MG combined detection was higher than that of single mAlb,β_(2)-MG diagnosis rate,the difference was statistically significant(P<0.05).The clinical positive rate of mAlb is higher thanβ_(2)-MG,the difference was statistically significant(P<0.05).The level of mAlb,β_(2)-MG at all ages gradually increased,and the difference was statistically significant(P<0.05).30-40 years old,41-50 years old,51-60 years old and mAlbβ_(2)-MG level has positive correlation(P<0.05).Conclusion The indexes of urinary mAlb andβ_(2)-MG are very important in the detection of diabetes mellitus.The combined detection of the two indexes can greatly improve the detection rate of kidney injury,indicating that the level of indexes is gradually improved with the increase of age,which is worthy of popularization and application.
