含ω-3鱼油脂肪乳的肠外营养支持对重型颅脑创伤患者营养状况的影响

Effect of parenteral nutrition support with omega-3 fish oil fat emulsion on nutritional status of patients with severe traumatic brain injury

摘要 目的:研究重型颅脑创伤患者采用含ω-3鱼油脂肪乳的肠外营养支持对其营养状况、体液免疫功能及肠道黏膜屏障功能的影响。方法:选取本院2021年2月至2022年2月期间103例重型颅脑创伤患者随机分组,对照组51例给予中/长链脂肪乳注射液的肠外营养,观察组52例给予含ω-3鱼油脂肪乳的肠外营养,于干预前和干预2 w后,采集两组患者晨间空腹静脉血5 mL,采用全自动生化分析仪检测并对比白蛋白(Albumin,ALB)、转铁蛋白(Transferrin,TRF)、血红蛋白(Hemoglobin,HGB)、前白蛋白(Prealbumin,PA)水平;于干预前和干预2 w后,采集两组患者晨间空腹静脉血5 mL,采用全自动生化分析仪检测并对比血清免疫球蛋白(Immune Globulin,Ig)IgG、IgM、IgA水平;于干预前和干预2 w后,采集两组患者晨间空腹静脉血5 mL,采用酶联免疫吸附法检测血清D-乳酸和内毒素水平。结果:与干预前相比,两组ALB、TRF、HGB、PA水平均提高(P<0.05),其中观察组更为显著(P<0.05);与干预前相比,两组IgG、IgM、IgA水平均提高(P<0.05),其中观察组更为显著(P<0.05);与干预前相比,两组D-乳酸、内毒素水平均降低(P<0.05),其中观察组更为显著(P<0.05)。结论:重型颅脑创伤患者采用含ω-3鱼油脂肪乳的肠外营养能够有效改善肠道黏膜屏障功能,提营养水平,增强体液免疫功能。 Objective:To study the effects of parenteral nutrition support containingω-3 fish oil fat emulsion on nutritional status,humoral immune function and intestinal mucosal barrier function in patients with severe traumatic brain injury.Methods:A total of 103 patients with severe traumatic brain injury in our hospital from February 2021 to February 2022 were randomly divided into two groups:51 patients in the control group were given parenteral nutrition with medium/long chain fat emulsion injection,and 52 patients in the observation group were given parenteral nutrition withω-3 fish oil fat emulsion.Fasting venous blood of 5 mL was collected from the two groups in the morning,and the levels of Albumin(ALB),Transferrin(TRF),Hemoglobin(HGB)and Prealbumin(PA)were detected and compared by automatic biochemical analyzer.Before intervention and 2 weeks after intervention,5 mL of fasting venous blood was collected in the morning of the two groups,and the levels of serum immunoglobulin(Ig)IgG,IgM and IgA were detected and compared by automatic biochemical analyzer.Before intervention and 2 weeks after intervention,5 mL fasting venous blood was collected in the morning of the two groups,and the levels of serum D-lactic acid and endotoxin were detected by enzyme-linked immunosorbent assay.Results:Compared with before intervention,the levels of ALB,TRF,HGB and PA in the two groups were increased(P<0.05),and the observation group was more significant(P<0.05).Compared with before intervention,the levels of IgG,IgM and IgA in the two groups were increased(P<0.05),and the observation group was more significant(P<0.05).Compared with before intervention,the levels of D-lactic acid and endotoxin in the two groups were decreased(P<0.05),and the observation group was more significant(P<0.05).Conclusion:Parenteral nutrition containingω-3 fish oil fat emulsion can effectively improve intestinal mucosal barrier function,improve nutritional level,and enhance humoral immune function in patients with severe traumatic brain injury.
