摘要
目的:探讨血栓弹力图(TEG)检测在产后出血患者凝血功能监测中的应用效果。方法:选取2020年2月—2022年1月广东省揭阳市妇幼保健计划生育服务中心收治的40例产后出血患者作为研究对象,均行凝血四项及TEG检测。观察患者常规凝血四项与TEG检测结果,比较两种检查方式低凝阳性率。结果:40例产后出血患者凝血四项中活化部分凝血活酶时间(25.68±1.24)s,凝血酶原时间(10.32±0.14)s,凝血酶时间(21.19±1.87)s,纤维蛋白原(4.49±0.52)g/L;TEG检测凝血因子反应时间(6.94±1.42)min,血凝块形成速率(1.87±0.74)min,Angle角(58.37°±6.96°),最大振幅(57.54±6.43)mm。TEG检测低凝阳性率高于常规凝血四项检测,差异有统计学意义(P=0.003)。结论:在产后出血患者凝血功能监测中积极应用TEG,可以在完成治疗的同时减少血液制品的消耗,缓解患者的经济压力。
Objective:To investigate the application effect of thrombelastogram(TEG)detection in monitoring blood coagulation function in patients with postpartum hemorrhage.Methods:A total of 40 patients with postpartum hemorrhage admitted to the Guangdong Jieyang Maternal and Child Health Care and Family Planning Service Center from February 2020 to January 2022 were selected as the study subjects.Four indicators of blood coagulation and TEG detection were performed in all patients.Four indicators of blood coagulation and TEG detection results were observed and the low coagulation positive rate of the two examination methods was compared.Results:For four indicators of blood coagulation in 40 patients with postpartum hemorrhage,activated partial thromboplastin time was(25.68±1.24)s,prothrombin time was(10.32±0.14)s,thrombin time was(21.19±1.87)s,and fibrinogen was(4.49±0.52)g/L.For TEG detection,blood coagulation factor reaction time was(6.94±1.42)min;blood clot formation rate was(1.87±0.74)min,Angle was(58.37°±6.96°),and maximum amplitude was(57.54±6.43)mm.The low coagulation positive rate of TEG detection was higher than that of conventional four indicators of blood coagulation detection,and the difference was statistically significant(P=0.003).Conclusion:The active application of TEG in the monitoring of blood coagulation function in patients with postpartum hemorrhage can reduce the consumption of blood products while completing the treatment,and relieve the economic pressure of patients.
