益肾消结方对阿霉素肾病综合征大鼠TGF-β1、SMAD3表达的影响

Effect of Yishen Xiaojie Recipe on the Expression of TGF-β1 and SMAD3 for Rats with Adriamycin Nephrotic Syndrome

摘要 目的:观察益肾消结方对阿霉素肾病综合征大鼠TGF-β1、SMAD3表达的影响。方法:选取雄性SD大鼠100只,适应性喂养1周后,随机分为健康组(20只)和模型组(80只),采用阿霉素注射法建立肾病综合征大鼠模型,去除造模后死亡大鼠9只,将造模成功大鼠随机分为模型对照组、治疗组、中药组各20只。健康组和模型对照组予生理盐水灌胃,治疗组予强的松+卡托普利片灌胃,中药组在治疗组的基础上加用益肾消结方颗粒灌胃,给药四周后,取尿液测量尿蛋白排出量,测量血清肝肾功、血脂等指标,取出大鼠肾组织行HE染色及PAS染色,分别观察肾小管及肾小球的组织形态结构变化,采用免疫组织化学检测大鼠肾小球中转化生长因子-β1(TGF-β1)及信号转导蛋白3(SMAD3)的变化及分布情况。结果:造模成功后,与健康组比较,模型组大鼠体质量均减轻,大鼠尿蛋白排出量均不同程度增加,光镜下可见模型组大鼠肾脏部分肾小球节段性硬化,肾脏皮质和髓质区域小管管腔内均可见蛋白管型的蓄积。灌胃4周后,与模型对照组比较,镜下可见治疗组及中药组大鼠损伤肾小球数量有所降低,小管内蛋白管型的蓄积均所减少;中药组大鼠尿蛋白排出量显著减少(P<0.05);中药组大鼠血清白蛋白显著升高(P<0.05),血清总胆固醇显著降低(P<0.01);与模型对照组比较,治疗组大鼠仅TGF-β1表达显著下调(P<0.05),SMAD3的表达无明显差异,而中药组大鼠TGF-β1、SMAD3表达较模型对照组则显著降低(P<0.01)。结论:益肾消结方具有减少尿蛋白排出、调节血脂及改善肾脏微循环的功效,可多途径参与下调TGF-β1、Smad3水平,达到延缓肾脏纤维化进程的作用。 Objective:To observe the effect of Yishen Xiaojie Recipe on the expressions of TGF-β1 and SMAD3 for rats with Adriamycin nephrotic syndrome.Methods:100 male SD rats were selected,and after 1 week of adaptive feeding,they were randomly divided into a healthy group(20 rats)and a model group(80 rats),and the nephrotic syndrome rat model was established by Doxorubicin injection.After the death of 9 rats,the successful model rats were randomly divided into the model controlled group,the treatment group,and the traditional Chinese medicine group,20 rats in each.The healthy group and the model controlled group were given normal saline by gavage,the treatment group was given Prednisone and Captopril by gavage,and the Chinese medicine group was given Yishen Xiaojie Granules by gavage on the basis of the treatment group.Urine was taken to measure urinary protein excretion,serum liver and kidney function,blood lipids and other indicators were measured.The rat kidney tissue was taken out for HE staining and PAS staining,and the changes in the tissue morphology and structure of renal tubules and glomeruli were observed respectively,and immunohistochemical detection was used.Changes and distribution of TGF-β1 and SMAD3 in rat glomeruli.Results:After the model was successfully established,compared with the healthy group,the body weight of the rats in the model group was reduced,and the urinary protein excretion of the rats increased to varying degrees.Under the light microscope,partialglomerular sclerosis in the kidneys of the rats in the model group was observed.Accumulation of protein casts was seen in the tubule lumen of both cortical and medullary regions.After 4 weeks of gavage,compared with the model controlled group,the number of damaged glomeruli in the treatment group and the traditional Chinese medicine group was decreased under the microscope,and the accumulation of protein casts in the tubules was reduced;the urinary protein excretion of the rats in the traditional Chinese medicine group significantly decreased(P<0.05);serum albumin was significantly increased(P<0.05)and serum total cholesterol was significantly decreased(P<0.01)in the traditional Chinese medicine group;compared with the model controlled group,the rats in the treatment group only expressed TGF-β1.There was no significant difference in the expression of SMAD3,while the expression of TGF-β1 and SMAD3 in the traditional Chinese medicine group was significantly lower than that in the model controlled group(P<0.01).Conclusion:Yishen Xiaojie Recipe has the effect of reducing urinary protein excretion,regulating blood lipids and improving renal microcirculation.It can participate in the down-regulation of TGF-β1 and Smad3 levels in multiple ways,so as to delay the process of renal fibrosis.
