摘要
目的:探究二甲双胍联合胰岛素泵短期强化标准化治疗对糖尿病合并妊娠患者的影响。方法:选取杭州钱塘区河庄街道社区卫生服务中心2020年10月-2021年9月收治的糖尿病合并妊娠患者80例,采用随机数字表法分为A组和B组,每组各40例,A组仅采用二甲双胍治疗,B组采用二甲双胍联合胰岛素泵短期强化治疗,比较两组患者治疗前、后血糖管理情况,及随访1年后的母婴不良结局。结果:治疗后,两组患者HbA1c、FPG、2h PG、HOMA-IR均降低,且B组低于同时期A组,同时两组患者FIns、HOMA-β均升高,且B组高于同时期A组,差异具有统计学意义(P<0.05);随访1年后,B组母婴不良结局总发生率低于A组(P<0.05)。结论:二甲双胍联合胰岛素泵短期强化标准化可以显著降低糖尿病合并妊娠患者的血糖水平、胰岛素抵抗作用,调节机体内胰岛β细胞功能,进而改善母婴结局。
Objective:To explore the effect of short-term intensive standardized treatment of metformin combined with insulin pump on diabetes patients with pregnancy.Methods:Eighty patients with diabetes complicated with pregnancy who were admitted to Hangzhou Qiantang District Hezhuang Street Community Health Service Center from October 2020 to September 2021 were randomly divided into group A and group B with 40 cases in each group.Group A was only treated with metformin,and group B was treated with metformin combined with insulin pump for short-term intensive treatment.The blood glucose management before and after,and adverse maternal and infant outcomes after a year’s follow-up between the two groups were compared.Results:After treatment,HbA1c,FPG,2h PG,HOMA-IR of patients in both groups decreased,and group B was lower than group A at the same time.At the same time,FIns,HOMA-βof patients in both groups in both groups increased,and group B was higher than group A at the same period,the difference was statistically signifi cant(P<0.05).After one year of follow-up,the total incidence of adverse outcomes of mothers and infants in group B was lower than that in group A(P<0.05).Conclusion:The short-term intensive standardization of metformin combined with insulin pump can signifi cantly reduce the blood glucose level and insulin resistance in pregnant women with diabetes,and enhance the islets in the body insulinβcell function,thereby improving maternal and infant outcomes.
