摘要
目的:探讨偏头痛的症状体征、中医证素分布特点及其相关性。方法:检索中国知识资源总库(中国知网)、中文科技期刊数据库(维普)、中国学术期刊数据库(万方)1998年2月1日-2021年7月19日老中医治疗偏头痛的文献。在完成筛选后建立数据库,提取其中医案信息,在中医传承计算平台(V3.0)上进行频数统计、聚类分析、关联规则分析,统计症状、舌脉、证候、治法频次,分析症状关联以及聚类效果。结果:纳入的文献共有91篇,包含145则医案。症状以头痛、睡眠差、恶心为多见;症状关联规则分析显示睡眠差-头痛、恶心-头痛、纳差-头痛的支持度较高;网络拓扑图显示核心症状为头痛、睡眠差、恶心等;聚类分析显示,聚为三类时聚类效果较好。舌脉频次统计显示以舌质暗、苔薄白、弦脉、细脉多见。中医证候以血瘀证、肝阳上亢证、风痰证最为多见;治法以通络止痛、活血化瘀、活血通络为主。结论:基于本研究筛选出的文献,偏头痛的中医证候以血瘀、肝阳上亢、风痰上扰为主,多见头痛、睡眠差、恶心、头晕,舌质暗苔薄白,脉弦滑等,治疗上以活血化瘀、通络止痛、疏肝解郁为主。
Objective:To explore the distribution characteristics of symptoms and signs,TCM syndrome elements of migraine and their correlation.Methods:The studies on treating migraine by veteran TCM practitioner from February 1,1998 to July 19,2021 were screened out in databases such as CNKI,VIP,and Wangfang data.After the screening,a database was established to extract the information of clinical cases included.The frequency statistics,cluster analysis,and association rule analysis were carried out by traditional Chinese medicine inheritance computing system platform(V3)to count the frequencies of symptom,tongue pulse,TCM syndrome,and treatment method,and to analyze the associations between symptoms,and clustering effects.Results:A total of 91 articles were included,involving 145 medical records.The most common symptoms were headache,poor sleep,and nausea;the analysis of symptom association rules showed that poor sleep-headache,nausea-headache,and anorexia-headache were highly supported,the network topology map showed that the core symptoms were headache,poor sleep,nausea,etc..The cluster analysis showed that the clustering effect was better when it was clustered into three categories.The tongue pulse frequency statistics showed that dark tongue,thin and white coating,stringy pulse,and thin pulse were more common.The most common TCM syndromes were blood stasis syndrome,liver Yang(阳)hyperactivity syndrome,and wind-phlegm syndrome.The main treatment methods were dredging collaterals and relieving pain,promoting blood circulation and removing blood stasis,and promoting blood circulation and dredging collaterals.Conclusion:Based on the studies screened out,the TCM syndromes of migraine are mainly blood stasis syndrome,liver Yang hyperactivity syndrome,and disturbance of wind-phlegm syndrome.The symptoms are mostly headache,poor sleep,nausea,dizziness,dark tongue,thin and white coating,stringy and slippery pulse,etc..The treatment is mainly promoting blood circulation and removing blood stasis,dredging collaterals and relieving pain,and soothing the liver and relieving depression.
