摘要
目的分析急性非静脉曲张性上消化道出血(ANVUGIB)患者的胃镜检查时间与治疗效果之间的关系。方法在广东省台山市人民医院消化专科门急诊于2021年4月-2022年10月收诊的ANVUGIB患者中随机抽取100例为对象,根据患者胃镜检查时间段不同分组:超早期组(入院后<8h内接受胃镜检查)、早期组(入院后8~24h内接受胃镜检查)、常规组(入院后24~48h内接受胃镜检查),对比三组患者的治疗效果指标。结果超早期组与早期组患者的活动性出血检出率、止血成功率、再出血率以及出血停止时间、住院时间比较差异均无统计学意义(P>0.05);常规组患者的活动性出血检出率、止血成功率均低于超早期组和早期组(P<0.05);常规组的再出血率高于超早期组和早期组(P<0.05);常规组的出血停止时间、住院时间均长于超早期组和早期组(P<0.05)。结论急性非静脉曲张性上消化道出血患者应尽早行胃镜检查,在入院后24h内接受胃镜检查,明确病因,快速止血,改善预后。
Objective To analyze relationship between time of gastroscopy examination and curative effect of patients with acute non-varicose upper gastrointestinal bleeding(ANVUGIB).Methods The paper chose 100 cases ANVUGIB patients in Gastroenterology Department and Emergency Department of Taishan City The People's Hospital of Guangdong from April 2021 to October 2022,and divided them into three groups according to time period of gastroscopy:ultra-early group(treated with gastroscopy within 8 hours after admission),early group(with gastroscopy within 8~24 hours after admission),and routine group(with gastroscopy within 24~48 hours after admission).Curative effect indexes of three group were compared.Results There was no significant difference in detection rate of active bleeding,success rate of hemostasis,rebleeding rate,time of stopping bleeding and length of stay between ultra-early group and early group,(P>0.05);However,detection rate of active bleeding and success rate of hemostasis in routine group was lower than ultra-early group and early group(P<0.05);rebleeding rate of routine group was higher than ultra-early group and early group(P<0.05);bleeding stop time and length of stay of routine group was longer than ultraearly group and early group(P<0.05).Conclusion It is necessary for patients with acute non-varicose upper gastrointestinal bleeding to carry on gastroscopy examination as soon as possible,and receive gastroscopy within 24 hours after admission,to clarify the cause,stop bleeding quickly and improve prognosis.
