两种验证系统在非均整模式下肺部立体定向放射治疗剂量验证中的应用 被引量:1

Application of Two Verification Systems in Dose Verification of Lung Stereotactic Body Radiation Therapy in Flattening Filter Free Mode

摘要 目的 比较瓦里安VitalBeam直线加速器自带的PD验证系统与IBA公司的Compass三维验证系统在非均整模式(FFF)下肺部立体定向放射治疗(SBRT)剂量验证中的应用价值。方法 选取2019年10月至2021年12月医院收治的的16例非小细胞肺癌(NSCLC)患者的FFF下SBRT计划,分别采用直线加速器自带的PD验证系统和Compass三维验证系统进行剂量验证,比较PD实测剂量、Compass计算剂量、Compass重建剂量的γ通过率,以及Compass计算剂量、重建剂量与治疗计划系统计算剂量的靶区、危及器官剂量偏差。结果 PD实测剂量、Compass计算剂量、Compass重建剂量的γ通过率均>90%;PD实测剂量的γ通过率均高于Compass计算剂量、Compass重建剂量,且Compass计算剂量的γ通过率高于Compass重建剂量,差异均有统计学意义(P<0.05)。CCD组(Compass计算剂量与治疗计划系统计算剂量比较)和CRD组(Compass重建剂量与治疗计划系统计算剂量比较)的靶区、危及器官剂量参数的剂量偏差比较,差异均有统计学意义(P<0.05),但偏差均在5%以内。结论 PD验证系统的验证效果优于Compass三维验证系统,而Compass三维验证系统的独立计算和实测重建功能可以全面分析理论照射剂量与实际照射剂量的差异,为精准放射治疗的实施提供依据。 Objective The application value of the PD verification system provided by Varian VitalBeam linear accelerator and the Compass 3D verification system of IBA Company in dose verification of lung stereotactic body radiation therapy(SBRT) under flattening filter free mode(FFF) was compared.Methods With selection of 16 patients with non small cell lung cancer(NSCLC) admitted to the hospital from October 2019 to December 2021for their SBRT plan under FFF,the dose verification was performed by using the PD verification system and the Compass 3D verification system,respectively.The γ passing rates of PD measured dose,Compass calculated dose and Compass reconstructed dose were compared,and the deviations between Compass calculated dose,reconstructed dose and target area and organ at risk dose calculated by the treatment planning system were compared.Results The γ passing rates of PD measured dose,Compass calculated dose and Compass reconstructed dose were all more than 90%;The γ passing rate of PD measured dose was higher than that of Compass calculated dose and Compass reconstructed dose,and the γ passing rate of Compass calculated dose was higher than that of Compass reconstructed dose,the differences were statistically significant(P<0.05).There were statistically significant differences in the dose deviation of the target area and organ endangering dose parameters between the CCD group(Compass calculated dose compared to the treatment planning system calculated dose) and the CRD group(Compass reconstructed dose compared to the treatment planning system calculated dose)(P<0.05),but the deviation was within 5%.Conclusion The verification effect of the PD verification system is superior to that of the Compass 3D verification system,while the independent calculation and measured reconstruction functions of the Compass 3D verification system can comprehensively analyze the differences between theoretical and actual irradiation doses,providing a basis for the implementation of precision radiotherapy.
