摘要
目的检测食管鳞癌患者癌组织中hMLH1与FHIT基因的mRNA及蛋白表达水平,并分析其与临床病理指标和预后之间的关系。方法收集60例食管鳞癌患者癌组织和癌旁正常组织标本,采用qRT-PCR法检测癌和癌旁组织中hMLH1和FHIT基因的mRNA表达情况,免疫组化法检测蛋白表达水平。χ^(2)检验分析hMLH1和FHIT蛋白表达与患者性别、年龄、肿瘤分化程度、淋巴转移、浸润深度等临床病理指标之间的关系,Spearman秩相关性分析hMLH1蛋白与FHIT蛋白表达之间的关系,并用Kaplan-Meier单因素和COX多因素分析影响食管癌预后的危险因素。结果qRT-PCR结果显示,在食管鳞癌组织中hMLH1、FHIT基因mRNA相对表达量显著低于癌旁正常组织(P<0.05)。免疫组化结果显示,hMLH1、FHIT蛋白在食管鳞癌组织中的阳性表达率显著低于癌旁正常组织(P=0.011)。hMLH1蛋白表达水平与淋巴结转移(P=0.007)、浸润深度(P=0.010)、病理分期(P=0.00)有关,而FHIT表达水平仅与浸润深度有关(P=0.013)。食管鳞癌组织中hMLH1和FHIT蛋白表达呈正相关(r=0.339,P=0.008)。Kaplan-Meier生存分析发现,食管鳞癌中hMLH1蛋白表达水平与患者总生存率相关(P=0.017)。COX多因素分析结果提示,肿瘤分化程度(P=0.00)和临床分期(P=0.001)是食管鳞癌患者不良预后的独立危险因素。结论食管鳞癌组织中hMLH1表达下降,这与食管癌患者预后不良有关,hMLH1与FHIT表达在食管癌恶性进展中可能存在协同作用。
Objective To investigate the mRNA and protein expression levels of hMLH1 and FHIT genes in esophageal squamous cell carcinoma,and explore their correlation with clinicopathological indicators and prognosis.Methods Esophageal squamous cell carcinoma tissues and adjacent normal tissues from 60 patients were collected to detect the mRNA and protein expression levels of hMLH1 and FHIT genes by RT-qPCR and immunohistochemistry,respectively.χ^(2) was used to analyze the relationships between the hMLH1 and FHIT protein expression and the clinicopathological indexes of patients such as sex,age,tumor differentiation,lymph metastasis,etc.,Spearman rank correlation analysis was used to analyze the correlation between the expression of hMLH1 protein and FHIT protein,and Kaplan-Meier single factor and COX multiple factor analysis were used to analyze the risk factors affecting the prognosis of esophageal cancer.Results RT-qPCR results showed that the mRNA expression of hMLH1 and FHIT in ESCC was significantly lower than that in normal adjacent tissues(P<0.05).IHC results showed that the positive expression rates of hMLH1 and FHIT proteins in ESCC were significantly lower than those in normal adjacent tissues(P=0.011).The expression level of hMLH1 protein was related to lymph node metastasis(P=0.007),depth of invasion(P=0.010)and pathological stage(P=0.00),but the expression level of FHIT protein was only related to the depth of invasion(P=0.013).The expression level of hMLH1 protein in ESCC was positively correlated with FHIT(r=0.339,P=0.008).Kaplan-Meier survival analysis showed that the expression of hMLH1 protein in ESCC was correlated with the overall survival rate of patients(P=0.017).COX multivariate analysis showed that the degree of tumor differentiation(P=0.00)and the clinical stage(P=0.001)were independent risk factors for poor prognosis in patients with ESCC.Conclusion Decreased expression of hMLH1 in esophageal squamous cell carcinoma tissues is associated with poor prognosis in patients with esophageal cancer.There may be a synergistic effect between hMLH1 and FHIT expression in the malignant progression of esophageal cancer.
