实时剪切波弹性成像与彩色脉搏波成像在2型糖尿病患者颈动脉硬度中的应用分析 被引量:1

摘要 目的探讨实时剪切波弹性成像与彩色脉搏波成像对2型糖尿病患者颈动脉硬度中的应用效果。方法选取东营市东营区中心医院2020年1月—2022年8月期间收治的94例2型糖尿病患者作为研究对象,以《中国2型糖尿病基层诊疗指南(2019年版)》中糖化血红蛋白(HbA1c)分层,分为3组,即A组(31例,HbA1c<6.5%)、B组(33例,HbA1c为6.5%~7.0%)、C组(30例,HbA1c为7.1%~8.0%)。比较3组2型糖尿病患者实时剪切波弹性成像检查结果和彩色脉搏波成像检查结果;比较2型糖尿病患者HbA1c水平与实时剪切波弹性成像、彩色脉搏波成像检查结果之间相关性。结果Min、Max、Mean在A组、B组、C组中依次升高,PWVBS、PWVES在A组、B组、C组中依次升高,差异有统计学意义(P<0.05),且上述指标均与HbA1c存在正相关关系。结论2型糖尿病患者在经实时剪切波弹性成像检查中,会出现Min、Max、Mean水平显著提升;应用彩色脉搏波成像检查中,会出现PWVBS、PWVES水平显著提升,且上述指标上升程度与患者血糖水平有关,提示可将其作为2型糖尿病患者颈动脉硬化情况评估指标,且若其指标上升程度明显,提示患者颈动脉硬化程度加重,可将其作为患者心脑血管并发症评估指标,并实施针对性防治,以保证患者生命安全。 Objective To investigate the application effect of real-time shear wave elastography and color pulse wave imaging in carotid artery stiffness in patients with type II diabetes mellitus.Methods 94 patients with type 2 diabetes admitted to Dongying District Central Hospital from January 2020 to August 2022 were selected as the study subjects.according to stratified glycated hemoglobin(HbA1c)stratification in the Chinese Primary Care Guidelines for Type 2 Diabetes(2019 edition),they were divided into 3 groups,namely,group A(31 cases with HbA1c<6.5%),group B(33 cases with HbA1c of 6.5%-7.0%),and group C(30 cases with HbA1c of 7.1%-8.0%).The results of real-time shear wave elastography and color pulse wave imaging in the three groups of type II diabetic patients were compared.Correlation between HbA1c levels and the results of real-time shear wave elastography and color pulse wave imaging in type II diabetic patients was compared.Results Min,Max,and Mean were increased in groups A,B,and C,and PWVBS and PWVES were increased in groups A,B,and C,and the differences were statistically significant(P<0.05),and all of the above indexes had positive correlation with HbA1c.Conclusion Patients with type II diabetes mellitus show significantly elevated levels of Min,Max,and Mean in real-time shear wave elastography.In color pulse wave imaging,PWVBS and PWVES levels were significantly increased,and the degree of increase of these indicators was related to the patient's blood glucose level,suggesting that they can be used as indicators for assessing carotid atherosclerosis in patients with type II diabetes mellitus.And if its indexes rise significantly,it suggests that the patient's carotid atherosclerosis degree is aggravated,and it can be used as an index to assess the patient's cardiovascular and cerebrovascular complications and implement targeted prevention and treatment to ensure the patient's life safety.
