孕妇碘营养状况与新生儿促甲状腺激素关系研究 被引量:1

摘要 目的:通过检测孕妇尿碘浓度(UIC)和新生儿甲状腺激素水平,研究孕妇在不同阶段碘营养状态与新生儿甲状腺激素的关系。方法:选择医院门诊接收的93例孕妇,孕妇都接受过规范化产检,将其划分为孕早期、中期、晚期三个组别,每个组别的样本数分别为26例、31例、36例,重点关注参与实验孕妇的UIC与甲状腺功能,把握住这些指标在不同阶段的变化情况,揭示其变化规律。同时,对入组孕妇进行追踪随访,检测分娩时新生儿TSH水平,并分析其与不同孕期UIC水平的关系。结果:三组孕妇的UIC、FT4、TSH水平无明显差异(P>0.05),孕晚期的FT3水平与前两个阶段相比有明显下降(P<0.001),孕早期UIC/UCr低于孕晚期(P=0.028)。UIC低下或正常的孕妇,新生儿TSH水平无明显差异(P>0.05)。结论:孕妇孕早期更容易出现碘缺乏,新生儿TSH水平与不同孕期出现的碘缺乏可能无对应关系。 Objective:To study the relationship between iodine nutritional status of pregnant women at different stages and neonatal thyroid hormone levels by detecting urinary iodine concentration(UIC)of pregnant women and neonatal thyroid hormone levels.Methods:A total of 93 pregnant women in the hospital were selected.According to the gestational age,they were divided into three groups:the first trimester group(26 cases),the second trimester group(31 cases)and the third trimester group(36 cases).At the same time,the enrolled pregnant women were followed up to detect the neonatal TSH level at delivery,and the iodine nutritional status and neonatal thyroid hormone levels were analyzed and compared among the three groups.Results:There were no significant differences in UIC,FT4 and TSH levels among the three groups(P>0.05).FT3 level in the third trimester was significantly lower than that in the first two stages(P<0.001),and UIC/UCr in the first trimester was lower than that in the first trimester(P=0.028).There was no significant difference in neonatal TSH level between mothers with low or normal UIC(P>0.05).Conclusion:Pregnant women are more likely to have iodine deficiency in the first trimester.There may be no corresponding relationship between neonatal TSH level and iodine deficiency in different trimesters.
