摘要
目的:通过诊断试验Meta分析,综合评价抗环瓜氨酸肽抗体(anti-cyclic citrullinated peptide antibody,Anti-CCP)、类风湿因子(rheumatoid factor,RF)及两者联合诊断类风湿关节炎(rheumatoid arthritis,RA),以获得综合准确度,用于评价诊断效果。方法:通过检索知网(CNKI)、万方、维普等数据库,搜集有关Anti-CCP抗体、RF及两者联合诊断RA的文献,提取相关的数据,采用RevMan5.4和Meta-Disc1.4进行诊断试验Meta分析。结果:Anti-CCP抗体对RA诊断的综合准确度为0.869,灵敏度为0.715,特异度为0.915,阳性诊断优势比为33.80;RF对RA诊断的综合准确度为0.8367,灵敏度为0.61,特异度为0.790,阳性诊断优势比为12.458;Anti-CCP和RF联合对RA诊断综的合准确度为0.9347,灵敏度为0.71,特异度为0.934,阳性诊断优势比为46.783。结论:Anti-CCP抗体诊断RA能够获得较好准确度的和特异度,但联合RF诊断RA可提高综合准确度和特异度,因此,建议两者联合用于RA诊断。
Objective:Anti-cyclic citrullinated peptide antibody(Anti-CCP),rheumatoid factor(RF)and the combination of both were evaluated by Meta-analysis of diagnostic tests to obtain a combined accuracy for the diagnosis of rheumatoid arthritis(RA).Methods:The literature on Anti-CCP antibodies,RF and the combination of the two for the diagnosis of RA was collected by searching databases such as Zhiwang(CNKI),Wanfang and Vipul.Relevant data were extracted.Meta-analysis of diagnostic tests was performed using RevMan 5.4 and Meta-Disc 1.4.Results:The combined accuracy of Anti-CCP antibody for the diagnosis of RA was 0.8694,sensitivity was 0.715,specificity was 0.915,and the positive diagnostic advantage ratio was 33.80.The combined accuracy of RF for the diagnosis of RA was 0.8367,sensitivity was 0.61,specificity was 0.790,and the positive diagnostic advantage ratio was 12.458.The combined accuracy of Anti-CCP and RF for diagnostic synthesis of RA was 0.9347,sensitivity was 0.71,specificity was 0.934,and the positive diagnostic advantage ratio was 46.783.Conclusion:Anti-CCP antibodies can achieve better accuracy and specificity in the diagnosis of RA,but the combination of RF in the diagnosis of RA can improve the combined accuracy and specificity.Therefore,the combination of both is recommended for the diagnosis of RA.
